Çocuklarda antibiyotikler en erken ne zaman ve hangi infeksiyonda başlanmaktadır?
Çocuklara antibiyotikler süt çocukluğu döneminden itibaren sık reçete edilmektedir. Bu kesitsel tanımlayıcı, retrospektif araştırmada, çocukların hayatlarında herhangi bir nedenle ilk kullandıkları antibiyotiklerin, indikasyonlarının ve ilk verilme yaşlarının belirlenmesi amaçlanmıştır. Çalışma 1 Mart 2004 ve 31 Temmuz 2004 tarihleri arasında farklı nedenlerle İstanbul Tıp Fakültesi Çocuk Polikliniği'ne başvuran 304 olgu ile yürütülmüştür. Araştırma döneminde toplam başvuru sayısı 3550 olgu idi (304/3550, % 9). Olgular ardışık sıra ile mesai saatleri içerisinde değerlendirmeye alınmıştır. Antibiyotik kullanımı ile ilgili bilgilere hasta dosyalarından, ilaç karnelerinden veya ailelerdeki eski reçete örneklerinden ulaşılmıştır. Antibiyotik kullanımına risk faktörleri olarak yuvaya gitme ile pasif sigara içiciliği sorgulanmıştır. İlk antibiyotik kullanma için yaş aralığı 10 gün -15 yaş, median 12 ay, antibiyotik kullanımına en sık neden olan endikasyon üst solunum yolu infeksiyorilan (% 53) ve ilk tercih edilen antibiyotikler sırasıyla ampisilin-sulbaktam; amoksisilin-klavulanik asit ve klaritromisin olarak saptanmıştır. Yuvaya gitme ve pasif sigara içiciliğinin araştırmamızda antibiyotik kullanımını etkilemediği görülmüştür.
The earliest time of life and the nature of infections for antibiotic usage in children
Antibiotics are commonly used in children beginning in infancy. In this retrospectivr and cross sectional study, the earliest age, indications and the kinds of antibiotics for the antibiotic usage were investigated in children. The research was conducted between 1st of March and 31st of July in 2004 with 304 cases applied to outpatient clinic of Istanbul Medical Faculty. The number of all applicants in study period was 3550 (304/3550, 9 %). Cases were admitted to study consequently in working hours. Patient's files, prescription pages of our drug data and historical prescription pages of the patient were used as data of antibiotics used before. Day care center and passive smoking were analyzed as risk factors for antibiotic usage. The time of first antibiotic usage varied from 10 days to 15 years, with a median of 12 months. The most frequent indication was upper respiratory tract infection (53 %) and the most frequently used antibiotics were ampicillin-sulbactam, amoxycillin-clavulanic acid and clarythromycin, respectively. Tenth day of life was determined as the earliest time of the first antibiotic usage and day care admittance and passive smoking were not determined as risk factors.
