Beş yılda izole edilen 15434 Escherichia coli ve 3178 Klebsiella spp. suşunda genişlemiş spektrumlu beta-laktamaz üretiminin yıllara, kliniklere ve örnek türlerine dağılımı

Son yıllarda infeksiyonlarda antimikrobiyal ajanlara direnç oranlarının arttığı gözlenmekte, bunlar arasında enterobakterilerin beta-laktam antibiyotiklere karşı geliştirdiği genişlemiş spektrumlu beta-laktamaz (GSBL) direnci önemli bir yer tutmaktadır. GSBL varlığı sıklıkla Escherichia coli ve Klebsiella spp. suşlarında saptanmaktadır. GSBL direnci plazmidler aracılığıyla türler arasında aktarılabilmekte, hastanelerde salgınlar oluşturabilmekte, tedavilerin yetersiz kalmasına ve hastaların hastanede yatış sürelerinin uzamasına ve mortalite oranlarının yükselmesine yol açabilmektedir. Bu çalışmada 2004-2008 yıllarında Acıbadem Hastaneleri ve Tıp Merkezlerinden gönderilen klinik örneklerde saptanan 15434 E.coli ve 3178 Klebsiella spp. suşunun GSBL oluşturma oranlarının yıllara, kliniklere ve örnek türlerine göre dağılımının incelenmesi amaçlanmıştır. Extended-spectrum beta-lactamase production in 15434 Escherichia coli and 3178 Klebsiella spp. strains isolated in five years according to years, departments and clinical samples

In recent years, extended-spectrum beta-lactamase (ESBL) resistance to beta-lactam antibiotics in Enterobacteriaceae strains gained more importance in the resistance to antimicrobial agents. ESBL presence is detected often in Escherichia coli and Klebsiella spp. and can be transferred by plasmids among species and causes outbreaks in hospitals, therapy failure, extended hospitalization and high mortality. In this study, the aim was to investigate ESBL production rates in 15434 E.coli and 3178 Klebsiella spp. strains isolated from clinical samples in Acibadem Hospitals and Medical Centers in regards with the years, departments and sample types during 2004-2008 years. The rates of ESBL producing E.coli strains have increased significantly each year (p<0.01-0.001), and from 3.8 % in 2004 reached to 17.2 % in 2008. For Klebsiella spp. strains these rates were similar in 2004 and 2005, but a significant elevation was observed in 2006 (from 7.7 % to 15.7 %, p<0.001) and stayed high (16.3 %) in 2007 and 2008.
