Pankreas başının lenfoepitelyal kisti: Vaka takdimi
Pankreasın lenfoepitelyal kistleri oldukça nadir görülen neoplastik olmayan kistlerdir. Vakaların çoğu asemptomatiktir ve diğer hastalıkların incelenmesi esnasında tanı konulur. Pankreasın lenfoepitelyal kistinin ayırıcı tanısı içerisinde müsinöz kistik neoplazi ve intraduktal papiller müsinöz neoplazi gibi pankreasın kistik neoplazileri veya psödokisti vardır. Pankreasın lenfoepitelyal kisti çok çeşitli görünümlerde olabilir, bu da ayırıcı tanıyı ve tedavi stratejisini güçleştirir. İnce iğne aspirasyon biyopsisi uygulanabilir ancak tanı şansı düşüktür. Biz burada bilgisayarlı tomografi taramasında rastlantısal olarak pankreas başında kistik kitle saptanan 54 yaşındaki erkek hastayı bildirmekteyiz. Kitlenin preoperatif malign veya benign ayırıcı tanısının yapılamaması nedeniyle hastaya eksploratif laparatomi uygulanmıştır. Kistik kitle pankreatikoduodenektomi ile pankreas başından rezeke edilmiştir. Patoloji incelemesi sonucu pankreasın lenfoepitelyal kisti olarak saptanmıştır.
Lymphoepithelial cyst of the pancreas head: Report of a case
Lymphoepithelial cysts of the pancreas are extremely rare, benign, nonneoplastic cysts. Most of the cases are asymptomatic and diagnosed during a work-up for other diseases. Diff erential diagnosis of lymphoepithelial cyst of the pancreas may include pseudocyst or cystic neoplasm of the pancreas such as mucinous cystic neoplasm and intraductal papillary mucinous neoplasm. Lymphoepithelial cyst of the pancreas has various imaging presentations, making the diff erential diagnosis and treatment strategy diff icult. Fine-needle aspiration biopsy should be considered but diagnosis is poor. Herein we report a 54-year old man who was admitted with a complaint of a cystic mass of the pancreatic head discovered incidentally in a computed tomography scan. Due to the lack of preoperative malign or benign diff erantial diagnosis of mass, the patient underwent a explorative laparatomy. The cystic mass was resected from the pancreatic head by pancreaticoduodenectomy. The final pathology was compatible with lymphoepithelial cyst of pancreas.
