Gastrointestinal sistemin karsinoid tümörleri ( Histopatolojik ve immünohistokimyasal bir çalışma )

Bu makalede gastrointestinal sistemde yerleşmiş 75 karsinoid tümör olgusunu sunduk. Çalışmamızda, nöroendokrin (chromogranin, neuron-spesific-enolase [NSE]) ve epitelyal (carcinoembryonic antigen [CEA]) belirleyicilerinden oluşan bir panel kullanılarak sindirim kanalının karsinoid tümörlerinin boyanma özellikleri belirlendi ve sonuçlar histolojik yapı, histokimyasal ve İmmunohistokimyasal boya sonuçları ile karşılaştırıldı. On erkek beş kadın hastadan oluşan serimizde yaş ortalaması 54 idi. Karsinoid sendromlu hastamız yoktu. Hastalarımızdan üçünde lenf bezi metastazı bulunmaktaydı. Olgularımız içerisinde mikst yapılı (glandüler alanlarla birliktelik gösteren insüler veya trabeküler alanlar) tümörler daha sık görülmekteydi. Yedi hastada tümör arjirofilik iki hastada arjantaffinikti. Neuron-spesifik-enolase ile pozitif boyanan 11 olgu bulunurken bütün hastalarda Kromogranin pozitif boyandı. CEA beş hastada pozitif boyandı. Bu çalışma temelinde; karsinoid tümörlerin tanısında, kullandığımız antikor panelinin yararlı olabileceği görülmektedir ve CEA'nın bulunduğu nöroendokrin tümörlerde hastanın prognozunun daha kötü olabileceği düşünülmektedir.

Digestive tract carcinoid tumors

In this article we presented fifteen digestive tract carcinoid tumors. In this study, a panel of neuroendocrine (NSE and Chromogranin) and epithelial (CEA) markers were used to determine patterns of staining in digestive tract carcinoid tumors, and results were compared with conventional histologic patterns and histochemical and immunhistochemical staining. The average age of the ten male and five female patients were 54. None of the patients had carcinoid syndrome. Lymph node metastases were present in three patients. Tumors of the mixed pattern (insular or trabecular with glandular areas) predominated. The tumors were arygrophilic in seven patients and argantaffinic in two patients. Neuron-spesific-enolase (NSE) was positive in eleven cases. Chromogranin was positive in all patients. CEA was positive in five cases. Based on this study, it appears that an antibody panel such as ours might be useful in diagnosing carcinoid tumors. The presence of CEA in neuroendocrine tumors may suggest that this patient' s prognosis may be poor.
