Evde yapılan penil blokaj sonrası gelişen konvülsiyon, kardiyak arrest ve ölüm (olgu sunumu)
Sirkümsizyon çocuklarda en sık uygulanan cerrahidir ve bu cerrahide sıklıkla subkütanöz halka blok uygulanmaktadır. Bu blok sırasında lokal anesteziklere ait majör bir sistemik toksik reaksiyon bildirilmemiştir. Bu makalede penis tabanına çevresel olarak yapılan subkütanöz prilokain infiltrasyonundan sonra gelişen bir konvülsiyon, kardiyorespiratuar arrest ve ölüm olgusu sunularak literatür ışığında konu irdelenmiştir. Sonuç olarak, sistemik toksisitenin özellikle çocuklarda her zaman olası olduğu, fakat toksisitenin kendisinden çok başlatacağı kısır döngü önemli olduğundan bu döngünün olabildiğince hızlı bir şekilde kırılması gerektiği, bu yüzden lokal anestezik güvenliği için oksijen, antikonvülzan ajan ve resüsitasyon ekipmanının hazır bulundurulması bir zorunluluk olduğu kararına varılmıştır.
Convulsion, cardiac arrest and death after penil blockade administrated in home (case report)
Circumcision is the surgical procedure most commonly performed on children and subcutaneous ring blockade is the main method used for this procedure. Any systemic toxicity following this anesthetic technique has not been reported. This report presents a case of convulsion, cardiac arrest and death in a child following penile blockade performed with prilocaine, and discusses the topic in the light of the literature. This case illustrates that systemic toxicity is always possible, especially in children, and the vicious cycle resulting from the toxicity is more important than the toxicity itself, so it is vital to break this cycle in as early stages. Therefore, it was decided that for the safety of local anesthesia, the availability of oxygen, anticonvulsants and resuscitation equipment should be made mandatory.
