Günübirlik el cerrahisi vakalarında preemptif analjezi: Tramadol, rofekoksib ve lornoksikam'ın karşılaştırıldığı randomize, kontrollü çalışma
Non-steroidal anti-inflamatuar ilaçların (NSAİİ) ve opioid-içeren analjeziklerin ameliyat öncesinde uygulanması, ameliyat sonrasındaki ağrı kesici ihtiyacını ve ağrıya karşı olan duyarlılıktaki artışı azaltabilir. Çalışmamızda 3 ayrı ilacın, median sinir blokajı altında el cerrahisi uygulanacak 40 ASA I-III [(ortalama yaş: 46 (16-70)] günübirlik hasta üzerindeki preemptif analjezik etkinliklerini karşılaştırdık. Ameliyattan 30 dakika önce; 100 mg tramadol (Grup T), 50 mg rofekoksib (Grup R), 8 mg lornoksikam (Grup L) ve placebo olarak da 675 mg Vit-C (Grup P) verildi. Hastalara evlerinde ağrıları olması halinde almaları için 500 mg parasetamol reçete edildi. Ağrı kesici etkinliği, ameliyat sonrasındaki 24 saat boyunca, belirlenen 5 ayrı zamandaki VAS değerleri öğrenilerek değerlendirildi. 0: etkili olmadı ve 5: ağrımı tamamen kesti olmak üzere 0 ile 5 değerleri arasında ağrı kesici etkinliği, toplam kullanılan parasetamol tablet sayısı, ürtiker, kaşıntı,ağız kuruluğu, bulantı, kusma, mide yanması, kabızlık, ishal, başağnsı ve baş dönmesi dahil olmak üzere yan etkilerin sıklığı ve ciddiyeti ile 0: memnun kalmadım ve 4: memnun kaldım olmak üzere 0 ile 4 arasında memnuniyet dereceleri de kaydedildi. Çalışma ilaçları arasında, preemptif analjezik etkinlikleri açısından istatistiksel olarak anlamlı bir fark bulunamadı. Lornok¬sikam grubu hariç diğer gruplarda, ameliyat sonrası 24 saat içerisindeki VAS değerlerine ait değişimler istatistiksel olarak anlamlı bulundu. The preemptive analgesia for ambulatory hand surgery: A randomized controlled pilot study to compare tramadol, rofecoxib and lornoxicam
The preemptive administration of NSAID's and opioid-containing oral analgesics may reduce postoperative analgesic requirements and hypersensitivity. We studied tine preemptive analgesic efficacy of three drugs in 40 ASA I-III patients (mean age: 46 (16-70), male:fenıale= 1:3) scheduled to undergo ambulatory hand surgery with median nerve blockade. 30 minutes before surgery, oral analgesic tablets were given asfo1lows: tramadol 100 mg, rofecoxib 50 mg, lornoxicam 8 mg and 675 mg Vitamin-C. Follow-up medication consisted of oral paracetamol 500 mg on demand. Analgesic efficacy was evaluated using a visual analogue scale at five different time intervals during the 24-hour postoperative study period. Global pain reliefon a 5-grade scale, the total number of analgesic tablets, the frequency and severity of adverse effects were recorded. The study drugs were not statistically different from each other for preemptive analgesic effects. In three of the groups, excluding Group L, the variation of VAS in the postoperative period was statistically significant. The 6"' hour records were statistically significant for Group L when compared to Group P (p=0.023), but not when compared to Groups R and T. Oral lornoxicam, tramadol and rofecoxib administered 30 minutes before surgery provide effective postoperative analgesia for ambulatory hand surgery.
