Çok erken başlangıçlı çocukluk çağı şizofrenisi: Bir olgu sunumu
Çok erken başlangıçlı şizofreni oldukça nadir görülen bir psikiyatrik bozukluktur. Çocukluk döneminde görülen diğer ruhsal bozukluklarla ayırıcı tanısının yapılması zor ve uzun zaman alan bir süreçtir. Bu yazıda otuz yedi aylıkken ilk psikotik belirtileri ortaya çıkan ve ilerleyen dönemlerde açık sanrı, varsam, dezorganize davranışları, konuşmaları gelişen ve çok erken başlangıçlı şizofreni tanısı ile izlenen bir erkek hasta sunulmuştur. Hastanın klinik özellikleri, tanı süreci ve ayırıcı tanısı literatür bilgileri ışığında tartışılmaya çalışılmıştır. Ayırıcı tanı çok erken başlangıçtı şizofreni ile duygudurum bozuklukları, gelişimsel dil bozuklukları, otizm spektrum bozuklukları ve organik etmenler arasında yapılmaya çalışılmıştır. Hastada gözlenen psikotik belirtilerin normal gelişimsel özelliklerle örtüşen yanları gözden geçirilmiştir. Antipsikotik tedavi ile belirtilerde kısmi düzelme olan hasta ilkokula başlatılamamış, ancak özel eğitim desteği alması sağlanabilmiştir. Otuz yedi aylıkken ani bir başlangıçla psikotik belirtilerin geliştiği bu olgu, literatürdeki en genç çok erken başlangıçtı şizofreni olgularından biriolarak bildirilmiştir.
Very early onset schizophrenia: a case report
'Very early onset schizophrenia' is a rare psychiatric disorder. Differential diagnosis of the disorder with other childhood psychiatric disorders is very difficult and time consuming. In this paper we reported the case of 37 months of age boy who suddenly developed delusions, hallucinations, dezorganized behaviors and diagnosed as very early onset schizophrenia. The clinical presentation, diagnostic evaluation and differential diagnosis were tried to discuss by literature knowledge. The differential diagnosis of very early onset schizophrenia was included affective disorders, developmental language disorders, autism spectrum disorders and organic diseases. Psychotic symptoms that interfere with normal developmental characteristics were reviewed. From the outset of this antipsychotic treatment partial remission was noted but the patient couldn't start an elementary school and he is now on a special education program. The case that developed psychotic symptoms suddenly at 37 months of age is reported as one of the youngest child with the diagnosis of very early onset schizophrenia.
