Sağlık hizmetleri finansmanında kullanıcı katkıları: Genel bir bakış vr Türkiye için bir durum değerlendirmesi

Çeşitli faktörlere bağlı olarak sağlık harcamaları artış eğilimi göstermektedir. Artan bu harcamalar kıt olan ülke sağlık finansman kaynakları üzerinde ciddi baskılar oluşturmaktadır. Genel vergiler ve zorunlu sosyal primlerle finanse edilen sağlık sistemlerinde, finansal yükü kamudan özele kaydırmak, ek gelir elde etmek, gereksiz talebi önlemek ve böylece harcamaları sınırlamak amacıyla kullanıcı katkıları kullanılmaktadır. Türkiye bir yandan sağlıkta dönüşüm programı ve sosyal güvenlik reformu ile sağlık güvencesi sunan sosyal güvenlik programlarını tek çatı altında birleştirerek oluşturduğu genel sağlık sigortası ile tüm nüfusu sağlık güvencesi kapsamına almayı amaçlarken, yani kapsam genişliğini artırmaya çalışırken, diğer taraftan çelişkili bir şekilde aynı reformlar kapsamında sağlık güvencesi kapsamının derinliğinin sığlaşmasına yol açan kullanıcı katkılarını da yaygınlıkla ve çeşitli yoğunluklarda kullanmaktadır. Bünyesinde hem fayda hem de zararları bir arada bulundurması nedeniyle kullanıcı katkılarından beklenen faydaların elde edilmesi ve olumsuz etkilerinin de en aza indirilmesi için yoksullar ve diğer kolay zarar görebilecek kesimler için koruma mekanizmalarının etkili ve güçlü bir biçimde sistemde kullanılması gerekmektedir.

User charges in health care financing: An overview and a situational analysis for Turkey

Health expenditures have shown an upward trend due to a range of factors. Increasing of the expenditures constitutes serious pressures on countries' scarce health financing resources. User charges have recently been used widely in various densities in order to shift the financial burden from public to private, gain additional revenue, avoid unnecessary demand and contain expenditures in health systems funded by general taxes and compulsory social contributions. On the one hand, within the health transformation program and social security reform, Turkey has been trying to ensure universal coverage establishing general health insurance scheme by merging all public social security programs under one roof, on the other hand, paradoxically, has widely been using user charges, which reduces the depth of coverage, in various densities within the coverage of the same reforms. Since user charges have both benefits and harms inherently, protection mechanisms must be employed effectively and powerfully for the poor and other vulnerable segments of the population in order to get expected benefits from user charges and minimize the negative effects of them.
