Apikal darlık, koledoğun bulbusa ektopik açılımı ve koledokogastrik fistül birlikteliği: olgu sunumu
Normalde ampulla vateri duodenum ikinci kıtasının posteromedial duvarında yer alır. Koledok ise duodenum ikinci kıtasının medial duvarı boyunca aşağıya doğru seyrederek buraya açılır. Koledoğun duodenum üçüncü ve dördüncü kıtasına veya daha proksimalde mideye, pilora ve bulbusa ektopik açılımı olabilmektedir. Safra yollarının mideye ektopik açılımı çok nadir görülmektedir. Mideye açılım en sık korpusa daha az sıklıkla antruma olmaktadır ve çoğunluğunda çift koledok bulunmaktadır. Bu yazıda apikal darlığı, koledoğun bulbusa ektopik açılımı ve koledokogastrik fistülü bulunan olguyu sunuyoruz.
Apical stenosis, ectopic opening of choledochus to duodenal bulb and choledochogastric fistulae togetherness: a case report
Normal localization for papilla of Vateri are commonly on the posterior wall in the second portion of duodenum, and choledochus lies down adjacent to the medial wall of the second portion of the duodenum, and opens into duodenum via papilla of Vateri. Choledochus can have proximal (stomach, pylorus and duodenal bulb) or distal (third or fourth portion of duodenum) ectopic openings beside this normal localisation. Ectopic opening of the biliary system into the stomach is quite rare. The most common site for gastric ectopic opening is the corpus and more rarely the antrum; in most cases there are double common bile ducts. Here we report a case with apical stenosis, ectopic opening of choledo- chus to duodenal bulb, and choledochogastric fistulae.
