Tek ve çift bant hamstring allogrefti ile ÖÇB rekonstrüksiyonlarının tünel genişlemesi ve izokinetik kas gücü açısından karşılaştırılması: Prospektif, randomize çalışma

Amaç: Çal›flmam›zda tek ve çift bant ön çapraz ba¤ (ÖÇB) rekonstrüksiyonu yap›lan dizlerin erken postoperatif dönemde tünel genifllemesi ve izokinetik kas gücü aç›s›ndan karfl›laflt›r›lmas› amaçland›. Çal›flma plan›: Çal›flmaya Kas›m 2007 - Mart 2008 aras›nda klini¤imizde ÖÇB rekonstrüksiyonu ya- p›lan, daha önce diz cerrahisi geçirmemifl 34 hasta al›nd›. Ortalama yafl› 27.3 (da¤›l›m: 19-35) olan 18 hastaya tek bant ÖÇB rekonstrüksiyonu, ortalama yafl› 30.1 (da¤›l›m: 20-40) olan 16 hastaya çift bant ÖÇB rekonstrüksiyonu yap›ld›. Tek veya çift bant yöntemine kurayla karar verildi. Tüm hastalara ameliyat öncesi hamstring ve kuadriseps izokinetik kas gücü de¤erlendirmesi Biodex 3 cihaz› ile yap›l- d›. Hastalar›n ameliyat sonras› 2. ve 3. ayda üç boyutlu diz eklem tomografileri çekildi. Alt›nc› aydaki kontrollerinde üç boyutlu diz eklem tomografisi ve izokinetik kas gücü de¤erlendirmesi tekrarland›. Her tünel alt› eflit parçaya bölünerek dijital ortamda sagital ve koronal planda tünel genifllik ölçümü, aksiyel planda ise ayn› noktalardan tünel alan ölçümü yap›ld›. Bulgular: Hem tek hem de çift bant ba¤ rekonstrüksiyonu yap›lan hastalar›n ameliyat öncesi ve son- ras›nda izokinetik kas gücü de¤erleri aras›nda farkl›l›k saptanmad›. Tek ve çift bant ÖÇB rekonstrük- siyonu sonras› 2., 3. ve 6. ayda çekilen tomografiler üzerinde yap›lan tünel çap ve alan ölçümleri ara- s›nda istatistiksel anlaml› fark bulunmad›. Ç›kar›mlar: ÖÇB cerrahisi sonras› erken dönemde tünel genifllemesi ve izokinetik kas gücü üzerine etkinli¤i aç›s›ndan tek ve çift bant yöntemleri benzer sonuçlar vermektedir.
