Erken Sakroiliit Tanısında Dinamik Manyetik Rezonans Görüntüleme ve Short Tau Inversion Recovery (STIR) Sekansının Karşılaştırılması

Amaç: Erken sakroiliit tanısında, dinamik manyetik rezonans görüntüleme(MRG) ile short tau inversion recovery (STIR) sekansı bulgularının karşılaştırılması ve klinik, laboratuar bulguları ile korelasyonunu değerlendirmeyi amaçladık. Gereçler ve Yöntem: Çalışmaya 2011-2015 yılları arasında Ankara Üniversitesi Tıp Fakültesi Fizik Tedavi ve Rehabilitasyon ile Romatoloji polikliniklerinden erken evre sakroiliit tanısıyla Radyoloji anabilim Dalımıza gönderilen 137 olgunun çekilen sakroiliak MRG görüntüleri dahil edildi. Sakroiliit açısından sakroiliak eklemlerin MRG incelemelerinde; subkondral skleroz, erozyon, periartiküler yağ birikimi, eklem aralığında daralma/ankiloz, kemik iliği ödemi (KİÖ)/Osteit, sinovit, entezit, kapsülit bulguları değerlendirildi. Sakroiliak eklem (SİE) S2 vertebra korpusundan geçen hayali çizgi ile süperior ve inferiora ayrıldı. SİE’i oluşturan eklem yüzeyleri sakral ve iliak olarak sınıflandırıldı. Her sakroiliak eklem 4 kadrana; yani bilateral olarak toplamda 8 segmente ayrıldı. Comparison of Dynamic Magnetic Resonance Imaging and Short Tau Inversion Recovery (STIR) at Determination in Early Sacroiliitis

Objective: We aimed to compare short tau inversion recovery (STIR) sequences and dynamic magnetic resonance imaging (MRI) in early diagnosis of sacroiliitis and to evaluate the correlation between them and laboratory findings. Material and Methods: One hundred thirty-seven patients who admitted to clinic of the Physical Therapy Rehabilitation and Rheumatology in Ankara University Faculty of Medicine between 2011-2015 were included. All of the patients were diagnosed with early sakroiliit. MRI images of all patients were included in the study. Subchondral sclerosis, erosion, periarticular fat accumulation, narrowing of the joint space / ankylosis, bone marrow edema (BME) / osteitis, synovitis, enthesitis, capsulitis symptoms were evaluated about sacroiliitis by MRI of the sacroiliac joint. Sacroiliac joint (SİJ) was separated superior and inferior by the imaginary line which passed through S2 vertebral body. Joint surfaces of SİJ were classified as sacral and iliac.
