Van Bölge Eğitim ve Araştırma Hastanesi'nde başlangıç laparoskopik cerrahi deneyimlerimiz
Amaç: Van Bölge Eğitim ve Araştırma Hastanesinde yapılan başlangıç laparoskopik cerrahi deneyimlerimizi geriye dönük olarak değerlendirmek. Gereç ve Yöntem: Kliniğimizde Eylül 2013 Ocak 2015 tarihleri arasında 25 (16sı erkek, 9u kadın) hastaya çeşitli laparoksopik ürolojik cerrahiler uygulandı. Operasyon süresi, kan kaybı, hastanede kalış süreleri, peroperatif ve postoperatif komplikasyonlar ve takip süreleri incelendi. Bulgular: Hastaların ortalama yaşı 38.6 (2- 69) idi. 11 hasta nonfonksiyone böbrek, 7 hasta böbrek kisti, 3 hasta üreter taşı, 3 hasta nonpalpabl testis ve 1 hasta da UPJ darlığı nedeniyle ameliyat edildi. Üreter taşları retroperitoneal, diğer vakalar ise transperitoneal olarak gerçekleş- tirildi. Ortalama operasyon süresi 97.6(20-150) dakika, hastanede kalış süresi 3.2 (1-6) gün, ortalama takip süresi 8.41 (1-16) ay olarak saptandı. Üreterolitotomi yapılan bir hastada taşın böbreğe migre olması sebebiyle açık operasyona geçildi. Hiçbir hastada kan transfüzyon ihtiyacı olmadı. Peroperatif veya postoperatif herhangi bir komplikasyon izlenmedi. Sonuç: Laparoskopik cerrahi daha az hastanede kalış süresi, daha az ağrı ve daha iyi kozmetik sonuçlar sebebiyle açık cerrahiye tercih edilebilir bir yöntemdir. Biz laparoskopik cerrahinin sadece üçüncü basamak sağlık kuruluşlarında değil, ikinci basamak sağlık kuruluşlarında da başarı ile uygulanabileceğini düşünüyoruz.
Our initial laparoscopic surgery experiences in Van Regional Training and Research Hospital
Objective: To retrospectively analyses our initial cases undergoing laparoscopic surgery in Van Regional Training and Research Hospital. Material and Methods: We performed various laparoscopic surgery 25 patients (16 male, 9 female) between September 2013 and January 2015 in our department. Operative time, blood loss, hospital stay, perioperative and postoperative complications and follow-up were evaluated. Results: The mean age of the patients was 38.6 (2-69) years. . The indications for surgery were non-functional kidney in 11 patients, renal cyst in 7 patients, , ureteral stone in 3 patients, nonpalpabl testis in 3 patients and UPJ obstruction in 1 patient. All laparoscopic ureterolithotomy were performed retroperitoneally. Other laparoscopic interventions were performed transperitoneally. The mean operative time was 97.6 (20-150) minutes. The mean hospital stay was 3.2 (1-6) days. The mean length of follow-up was 8.41 (1-16) months. Open surgery conversion was indicated only the one patient due to stone migration, who was performed ureterolithotomy. None of the patients did not require blood transfusion. No peroperative and postoperative complication was observed Conclusions: Laparoscopic surgey is a preferable method than open surgery with the shorter hospital stay, the less postoperative pain relief and the more favourable cosmetic results. We believe that, the laparoscopic surgery does not only third step hospital, but also can apply successfully in the second step hospital.
