Serebral palsi ve ileri derecede kifoskolyozu olan çocuk hastada unilateral spinal anestezi

Serebral palsi genellikle yaşamın ilk yılında beyine olan bir hasardan kaynaklanan ve motor fonksiyonda hasarlanmaya yol açan, ilerleyici olmayan bir bozukluktur. Kifoskolyoz, solunum sisteminde meydana getirdiği olumsuz değişiklikler, olası ventilasyon problemi ve entübasyon güçlüğü nedeni ile anestezi açısından önemli morbidite riski taşımaktadır. Biz ileri derecede kifoskolyozu ve serebral palsisi, beraberinde restriktif akciğer hastalığı olan, kalça artroplastisi yapılan olguda, 7.5 mg hiperbarik bupivakain ile (%0,5) unilateral spinal anestezi uyguladık. Gerek peroperatif dönemde ve gerekse postoperatif dönemde önemli bir komplikasyon gözlenmedi. Sonuç olarak, unilateral spinal anestezinin serebral paralizili hastalarda, alt ekstremite cerrahisinde alternatif bir anestezi tekniği olabileceği düşüncesindeyiz.

Unilateral spinal anesthesia in a pediatric patient with cerebral palsy and severe kyphoscoliosis

Cerebral palsy a non progressive disorder resulting from an injury to the brain, usually within the first year of life, and resulting in impairment in motor function. Kyphoscoliosis, with pulmonary complications, possible ventilation problems and intubation difficulties, has high risks of morbidity for anesthesia. We performed unilateral spinal anesthesia with 7.5 mg of hyperbaric bupivacaine (0.5%) in a severely kyphoscoliotic and cerebral palsy patient who was suffering from restrictive pulmonary disease and underwent hip arthroplasty operation. There was no complication in either the peroperative or postoperative period. In conclusion, unilateral spinal anesthesia may be an alternative anesthesia technique in cerebral palsy patients undergoing lower extremity surgery.
