Özofagusta yabancı cisim saptanan olgularımızın retrospektif analizi

Giriş: Özofagus yabancı cisimleri (ÖYC) çocuklarda ve erişkin yaş grubunda sık olarak karşılaşılan ve aynı zamanda ciddi morbidite ve mortaliteye neden olabilecek önemli bir sorundur. Bu nedenle ÖYC’lerin erken tanı ve tedavisi hayatı tehdit eden komplikasyonların önlenmesi için çok önemlidir. Biz bu çalışmamızda özefagus yabancı cisimleri nedeniyle kliniğimizde acil endoskopi işlemi uyguladığımız hastaların retrospektif analizini yapmayı hedefledik. Materyal ve Metot: Şubat 2008 ile Ocak 2011 tarihleri arasında Türkiye Yüksek İhtisas Hastanesi’nde özefagus yabancı cisimleri ön tanısıyla endoskopi uygulanan hastaların yaşı, cinsiyeti, yabancı cisim tipi, yabancı cismin saptandığı lokalizasyon, uygulanan endoskopik tedavi ve başvuru semptomları çalışma için hazırlanan özel formlara kaydedildikten sonra veriler istatistiksel olarak analiz edildi. Bulgular: Çalışmaya dahil edilen 60 hastanın 28’i (%46,6) erkek 32’si (%54,4) kadındı. Erkeklerde ortalama yaş 53,2±18,7 kadınlarda 41,0±17,4 idi. En sık saptanan yabancı madde 40 (%66,7) olguda gıda artığı idi. Bunu sırasıyla 15 (%25) hasta ile metalik madde, 5 (%8,3) hasta ile kabuklu meyve ve/veya çekirdeği izliyordu. En sık başvuru nedeni sırasıyla 26 (%43,3) hastada yutma güçlüğü, 19 (%31,7) hastada boğaz ağrısı, 15 (%25) hastada boğazda takılma hissi olarak tespit edildi. Endoskopik işlem sonrası 2 hasta cerrahiye yönlendirildi. Tartışma: Özofageal yabancı cisimlerin erken tanı ve tedavisi, ciddi ve hayatı tehdit edebilecek komplikasyonlar ın önlenmesi açısından önemlidir. Erken dönemde uygulanan üst gastrointestinal sistem endoskopisi günümüz şartlarında en etkili ve güvenilir tedavi yöntemidir.

Retrospective analysis of esophageal foreign bodies: A single center experience

Introduction: Esophageal foreign body aspiration is a common event which can cause serious morbidity and mortality in the children and adult population. For that reason, early diagnosis and treatment are crucial for preventing these life threatining complications. In this study, we aimed to analyse the characteristics of the patients with esophageal foreign bodies that were treated in our department. Materials and Methods: Patients diagnosed with esophageal foreign bodies who were admitted to Türkiye Yüksek Ihtisas Training and Research Hospital between February 2008 and January 2011 were evaluated retrospectively. The data regerding their age, gender, clinical profile, type and localization of the esophageal foreign body, performed endoscopic procedure and inital symptoms of the patients were noted and analyzed statistically. Results: Sixty patients with a diagnosis of esophageal foreign body were included in this study. Of these patients, 28 (46.6%) were male and 32 (54.4%) were female. Mean age was 53.2±18.7 in males, and 41.0±17.4 in females. Most of the foreign bodies (66.7%) detected in the esophagus was food waste. Metallic coins and fruit stones were detected in 15 (25%) and 5 (8.3%) patients respectively. The most common complaint was dysphagia (43.3%). Other reported complaints were sore throat (31.7%) and sense of food impaction on throat (25%). After endoscopic intervention two of the patients were directed to surgery. Discussion: Early recognition and treatment of esophageal foreign bodies is important as their complications are life threatining. Early management with upper gastrointestinal endoscopy is the most efficient and safe treatment method in current conditions.
