Hemodiyaliz hastalarında hipertansiyon

Amaç: Hemodiyaliz ünitesinde kan basıncı ölçümü sırasında pek çok ölçüm hatası ve hatta beyaz önlük etkisi gözlemlenebilir. Çalışmamızla hemodiyaliz hastalar ındaki kan basıncı ölçümlerinde beyaz önlük etkisinin önemi vurgulanmıştır. Materyal ve Metot: Bu çalışmaya düzenli hemodiyaliz uygulanan yetmiş dokuz hasta dahil edildi. Kan basınçlar ı, hemodiyaliz öncesi dinlenme salonunda ve diyalize başlarken ölçüldü. Bulgular: Dinlenme salonunda ölçülen sistolik ve diyastolik kan basıncı ortamaları sırasıyla 128,22±26,38 mmHg ile 74,55±15,91 mmHg ve hasta hemodiyaliz ünitesine alındığında ölçülen sistolik ve diyastolik kan basıncı ortalamaları ise 135,94±31,52 mmHg ile 79,36± 18,96 mmHg bulduk (sırasıyla;p

Hypertension in hemodialysis patients

Objective: The measurement of blood pressure (BP) in the hemodialysis (HD) units is subject to a number of observer errors and also to the white-coat effect. Our study highlights the importance of the white-coat effect to the measurements of BP in the HD patients. Material and Methods: Seventy-nine patients who were undergoing HD were included in the study. BP measurements were performed before HD in the waiting room and just before the HD. Results: We found that the average waiting room systolic and diastolic BP’s were 128,22±26,38 mmHg and 74,55±15,91 mmHg, respectively and that the average hemodialysis unit systolic and diastolic BP were 135,94±31,52 mmHg and 79,36±18,96 mmHg, respectively (p<0.0001 and p=0,0002, respectively). Conclusion: The white coat effect is highly prevalent among the patients undergoing HD. In these patients, pursuing a low BP target especially at the dialysis session and nighttime with intensified anti-hypertensive therapy may not be safe due to the risk of cardiohypoperfusion.
