Yüzeyel femoral arter ve ven arasındaki travmatik fistülün stent-greft ile tedavisi

Otuz üç yaşındaki erkek olguda, 2,5 yıl önce bıçaklanma sonucu sol yüzeyel (süperfisyel) femoral arter (SFA) ve yüzeyel (süperfisyel) femoral ven (SFV) arasında gelişmiş travmatik arteriovenöz fistülün, dijital subtraksiyon anjiyografi (DSA) bulgularını sunmayı ve endovasküler yolla (stent-greft ile) tedavi sonuçlarını bildirmeyi amaçladık. Endovasküler stent-greft yerleştirilerek arteriyovenöz fistülün tam olarak kapanması sağlandı. Tedaviden 22 ay sonra yapılan en son DSA, sol SFA’nın stent lokalizasyonunda tam oklüzyona uğradığını, bu bölgede çok sayıda kollateral geliştiğini ve bunların sol SFA’nın distalini, herhangi bir dolaşım bozukluğu ortaya çıkmadan doldurduğunu gösterdi. Özellikle seçilmiş olgularda, daha az ağrıya ve hareket kaybına yol açması, daha az maliyetli olması ve hastaneden daha çabuk taburcu olunmasını sağlaması, endovasküler stent-greft tedavisini standart cerrahi tedaviye tercih edilebilir kılmaktadır. Son yıllardaki deneyimler ve hasta takip bilgileri, bu tedavinin az sayıda komplikasyonu olan, uygun bir yöntem olduğunu göstermektedir.

Management of a traumatic fistula between superficial femoral artery and vein with stent-graft treatment

The purpose of the study was to present digital subtraction angiography (DSA) images of a traumatic arteriovenous fistula (AVF), which developed between left superficial femoral artery (SFA) and superficial femoral vein (SFV) in a 33 year-old-man, who was stabbed 2,5 years ago and also to report the outcome of its treatment with endovascular stent-graft placement. With the placement of endovascular stent graft, successful closure of the AVF was achieved. The last control with DSA, performed 22 months after the treatment, revealed total occlusion of the left SFA at the location of the stent, with development of collateral arteries which provide the continuity of blood flow through the distal portion of SFA without any compromise of the distal circulation. Endovascular stent graft treatment of AVFs, causing less pain and disability, and providing lower cost and rapid discharge from the hospital, can be compared to the standard surgical techniques, especially in selected cases. Recent experience and follow up data show that it is a favourable method with few complications.
