Dev kolon polibine bağlı kolokolik invajinasyon ve obstrüksiyon: Bir olgu

Kolon polipleri kolonoskopide sık karşılaşılan bir durum dur. Çoğunlukla bir yakınmaya sebep olmazlarken, nadi ren karın ağrısı, rektal kanama, invajinasyon ve barsak tıkanıklığına yol açabilirler. Çocuklarda kolon polibine bağlı invajinasyon görülmekle beraber yetişkinlerde bu tür poliplere bağlı invajinasyon ve obstrüksiyon nadir görülen bir durumdur. Sunduğumuz olgu, uzun süredir kabızlık yakınması olan bir bayandır. Yakınmasının arttığı bir gün barsak tıkanıklığı tanısı ile hastaneye yatırılmış ve tetkikleri sonucunda sigmoid kolonda invajinasyon ve tıkanıklığa neden olmuş dev kolon polibi tesbit edilmiştir. Polibin boyutunun büyük olması nedeniyle kolonoskopik yolla girişim yapılamayan hastaya ameliyat uygulanmış ve segmenter kolon rezeksiyonu ile tedavi yapılmıştır. Hasta ameliyat sonrası sorunsuz taburcu edilmiştir. Polibin histolojik tanısı benign olarak bildirilmiştir. Benign poliplerin bu derece büyüyüp invajinasyona ve tıkanık lığa sebep olması nadir rastlanılan bir durum olduğundan olgumuz önemlidir.

Colocolic invagination and obstruction caused by a giant colonic polyp: a case report

Polyps in the colon are commonly seen in colonoscopy. In general, they are asymptomatic, but complaints like abdominal pain, rectal bleeding and invagination- obstruction can be caused by polyps. Invagination and obstruction due to colonic polyps can occur in children but rarely in adults. In this case report, we present a woman with long-term constipation who was admitted to the hospital with colonic obstruction after worsening of her symptoms. After diagnostic testing she was found to have a polyp in the sigmoid colon that caused colocolic invagination and obstruction. Due to the large size of the polyp, a colonoscopic intervention could not be performed. As a result, the patient had segmentery colonic resection and anastomosis surgery. The patient was discharged following the surgery without any complication. The polyp was reported to be benign with the histologic study. This case is important because benign polyps rarely grow to a size that can cause colonic invagination and obstruction.
