Anti-CCP Antibodies in Behcet's Disease with or without articular Involvement

Amaç: Eklem tutulumu Behçet hastalığının (BH) önemli bir bileşenidir ve bazen ayırıcı tanıyı zor hale getiren tek başlangıç bulgusu olabilir. Siklik sitrüline peptid (CCP)’e karşı otoantikorlar RA için yüksek özgüllükleri nedeniyle özellikle dikkati çekmektedirler. Çalışmanın amacı eklem tutulumu olan veya olmayan BH ile anti-CCP ilişkisini belirlemek ve sonuçları romatoid artrit (RA) ve osteoartrit (OA) ile karşılaştırmaktır. Materyal ve Metot: Anti-CCP antikorları BH olan 30 hastada, RA olan 16 hastada ve OA olan 14 hastada ölçüldü. 30 BH hastasının onaltısında artrit vardı, geri kalanlarda hiçbir eklem tutulumu yoktu. Bulgular: BD tanısı ve aynı zamanda artriti olan sadece bir hastada (%3.3), anti-CCP pozitifliği vardı. RA olan hastaların onbiri (69%) ve OA olanların birinde (%7) anti-CCP antikor düzeyi katof değerin üstünde idi. Anti- CCP antikor düzeyi ile BH (P>0,05) ve OA (P>0,05) arasında istatistiksel olarak anlamlı ilişki yoktu ancak RA, BH ve osteoartritten anlamlı şekilde ayırt edilebilir (p< 0.01). Sonuç: Anti-CCP antikorlarının BH ve eklem tutulumunda tanısal değeri yoktur ve sonuçlar noninflamatuar bir eklem hastalığı olan OA’den farklı değildir. Ileri çalışmalara ihtiyaç vardır, ancak biz bu otoantikorların RA ve BH artriti ayırıcı tanısında kullanılabileceğini düşünmekteyiz.

Eklem tutulumu olan veya olmayan Behçet hastalarında anti-CCP antikorları

Objective: Articular involvement is an important component of Behcet’s disease, which occasionally can be the only initial manifestation that makes the differential diagnosis difficult. Autoantibodies to cyclic citrullinated peptide are particularly remarkable because of their high specificity for RA. The objective of the study was to determine the association of anti-CCP in BD with or without articular involvement, and to compare the results with rheumatoid arthritis and osteoarthritis (OA). Methods: Anti-CCP antibodies were measured in 30 patients with BD, 16 patients with RA, and 14 with OA. Sixteen of 30 BD patients had experienced arthritis, the remainder had no articular involvement. Results: Only one (3.3%) patient diagnosed with BD, who also suffered from arthritis, had anti-CCP positivity. Eleven (69%) patients with RA and one (7%) with OA had anti-CCP antibodies above the cut-off level. There was no statistically significant correlation between anti- CCP antibodies and BD (P>0.05), and OA (P>0.05), however, RA can be distinguished significantly from BD and osteoarthritis (P<0.01). Conclusions: Anti-CCP antibodies have no diagnostic value in BD and its articular involvement, and the results are not different from OA which is a noninflammatory articular disease. Further studies are needed, but we believe that these autoantibodies can be used in differential diagnosis of RA and BD arthritis.
