AYNI ARTER KULLANIMI İLE ARDIŞIK SKAPULAR SERBEST FLEP VE ‘TOE TO THUMB’ TRANSFER İLE FONKSİYONEL EL REKONSTRÜKSİYONU: OLGU SUNUMU
Giriş: Endüstriyel sektör çalışanlarında komplike, ezici el yaralanmaları sıkça görülmektedir. Bu tür yaralanmalarda ele fonksiyonunu kısmen de olsa geri kazandırabilmek, uzun tedavi süreçleri ve birden çok aşamalı cerrahi prosedürler uygulanmasını gerektirmektedir. Olgu Sunumu: Burada, ciddi ezici el yaralanması olan bir olguda, alıcı olarak aynı arter kullanılarak ardışık seanslarda serbest skapular flep ve toe to thumb prosedürlerinin uygulanması başarılı bir fonksiyonel restorasyon sağlanmıştır. Tartışma: Rekonstrüktif el cerrahisi; tanımlanmış kompleks prosedürler sayesinde ele yeniden kabul edilebilir fonksiyon sağlayabilmektedir. Bu nedenle, ciddi yaralanması olan bir elde tedavinin ilk aşamalarında amputasyon kararı; sadece onarımın mümkün olmadığından emin olunduğu koşullarda verilmelidir
FUNCTIONAL HAND RECONSTRUCTION WITH SEQUENTIAL SCAPULAR FREE FLAP AND TOE TO THUMB TRANSFER BY USING THE SAME ARTERY: CASE REPORT
Introduction: Complicated hand injuries are common among industrial sector workers. Multi-staged surgical procedures and long-term treatment processes are required for these patients in order to partially restore the function of the hand. Case Report: Here in a patient with a serious hand injury, successful functional restoration was provided through a free scapular flap transfer and toe to thumb transfer using the same recipient artery in consecutive sessions. Conclusion: Today, reconstructive hand surgery procedures can provide acceptable functional results even after serious hand injuries. For this reason, decision of amputation of a hand in the early stages of treatment should limited solely to specific non-repairable cases.
