Yoğun bakım ünitelerinde yatan hastaların kan kültürlerinden izole edilen mikroorganizmalar ve antibiyotik duyarlılıkları
Amaç: Kan dolaşımı enfeksiyonlarına yol açan etkenlerin ve antibiyotik duyarlılıklarının tanımlanması ve belirlenmesi, klinisyeni doğru ve uygun ampirik tedaviye yönlendirmek açısından önemlidir. Bu çalışmada, hastanemiz yoğun bakım ünite (YBÜ)'lerinde yatan hastaların kan kültürlerinden izole edilen mikroorganizmaların dağılımının ve antibiyotik duyarlılıklarının incelenmesi amaçlanmıştır.Yöntem: Kan örnekleri BacT/ALERT 3D (bioMérieux, Fransa) otomatik kan kültürü sisteminde inkübe edilmiştir. İzole edilen bakteri suşlarının tanımlanması ve antibiyotik duyarlılık testleri Vitek 2 compact (bioMérieux, Fransa) otomatize sistemi ile yapılmıştır. Vankomisin, teikoplanin, linezolid, imipenem ve meropenem direnci E-test (bioMérieux, Fransa) yöntemiyle doğrulanmıştır. Maya mantarlarının tanımlanması ve antifungal duyarlılık testleri için API ID 32C ve API ATB Fungus 3 (bioMérieux, Fransa) kitleri kullanılmıştır.Bulgular: İzole edilen 829 mikroorganizmanın 372 (%44.9)'si gram pozitif, 334 (%40.3)'ü gram negatif bakteri, 123 (%14.8)'ü maya mantarı olarak tanımlanmıştır. En sık izole edilen etkenler; sırasıyla koagülaz negatif stafilokoklar (KNS) (%25.3), Enterococcus spp. (%13.6), Acinetobacter spp. (%13.1), Candida parapsilosis (%8.3), Escherichia coli (%7.9), Klebsiella spp. (%7), Staphylococcus aureus (%4.9), Pseudomonas aeruginosa (%4.8), Candida albicans (%4.7), Serratia marcescens (%2.8), Proteus spp. (%1.8), ve Enterobacter spp. (%1.7) olarak belirlenmiştir. KNS suşlarının %79.5'u, S. aureus suşlarının %12.2'si metisiline dirençli olarak saptanmıştır. Stafilokok suşları ve Enterococcus faecalis'de glikopeptid ve linezolid direncine rastlanmazken, Enterococcus faecium suşlarında %15.5 vankomisin, %13.8 teikoplanin ve %1.7 linezolid direnci görülmüştür. Karbapenem direnci Acinetobacter baumannii'de %90.4, P. aeruginosa'da %45, S. marcescens'de %8.7 ve Klebsiella spp.'de %8.6 olarak bulunurken, kolistin direncine rastlanmamıştır. C. parapsilosis en sık izole edilen maya türü olarak saptanırken, Candida türlerine karşı en etkili antibiyotikler flusitozin ve amfoterisin B olarak bulunmuştur.Sonuç: Hastanemiz YBÜ'lerinde yatan hastaların kan kültürlerinden vankomisine dirençli enterokokların, çoklu antibiyotik direnci gösteren A. baumannii, P. aeruginosa ve K. pneumoniae suşlarının izole edilmesi; daha etkin enfeksiyon kontrol programlarının ve akılcı antibiyotik kullanım politikalarının uygulanması gerektiğini göstermiştir.
Microorganisms isolated from blood cultures of the patients in intensive care units and their antibiotic susceptibilities
Objective: The identification and determination of antibiotic susceptibilities of the agents causing bloodstream infections is of importance in respect of guiding the clinician to lead a correct and appropriate empirical therapy. The aim of this study was to investigate the distribution and antibiotic susceptibilities of microorganisms isolated from the blood cultures of patients in our hospital intensive care units (ICUs).Methods: Blood samples were incubated in the BacT/ALERT 3D (bioMérieux, France) automated blood culture system. Identification and antibiotic susceptibility testing of the isolated bacterial strains were performed by Vitek 2 compact (bioMérieux, France) automated system. Vancomycin, teicoplanin, linezolid, imipenem and meropenem resistance was confirmed by E-test (bioMérieux, France) method. API ID 32C ve API ATB Fungus 3 (bioMérieux, France) kits were used for the identification and antifungal susceptibility testing of the yeasts.Results: Of the isolated 829 microorganisms, 372 (44.9%) of them were identified as gram positive, 334 (40.3%) gram negative bacteria and 123 (14.8%) yeasts.The most frequently isolated agents were determined as coagulase-negative staphylococci (CNS) (%25.3), Enterococcus spp. (%13.6), Acinetobacter spp. (%13.1), Candida parapsilosis (%8.3), Escherichia coli (%7.9), Klebsiella spp. (%7), Staphylococcus aureus (%4.9), Pseudomonas aeruginosa (%4.8), Candida albicans (%4.7), Serratia marcescens (%2.8), Proteus spp. (%1.8), and Enterobacter spp. (%1.7), respectively. Methicillin resistance was found in 79.5% of CNS and 12.2% of S. aureus strains. No glycopeptide and linezolid resistance was found in staphylococci and Enterococcus faecalis strains, and vancomycin, teicoplanin and linezolid resistance in Enterococcus faecium strains was determined as 15.5%, 13.8% and 1.7%, respectively. While carbapenem resistance was found as 90.4% in Acinetobacter baumannii, 45% in P. aeruginosa, 8.7% in S. marcescens and 8.6% in Klebsiella spp. and there was no resistance to colistin. C. parapsilosis was the most commonly isolated yeast species when flucytosine and amphotericin B were found to be the most effective antibiotics for Candida species.Conclusion: The isolation of vancomycin-resistant enterococci, multi-antibiotic resistant A. baumannii, P. aeruginosa and K. pneumoniae strains from the blood cultures of patients in our hospital ICUs has indicated that more effective infection control programs and rational antibiotic use policies should be implemented.
