Although surgical intervention is considered to be the mostappropriate therapeutic choice for acute type 1 dissectioneven in asymptomatic cases, the diameter of ascendingaorta is critical for decision in chronic asymptomaticcases. In this article, we report a 50-year-old male caseof chronic type 1 dissection with left main coronaryartery originating from the right coronary sinus. Acutetype 1 dissection probably occurred two months ago andthe patient was misdiagnosed with pulmonary edema.During surgery, the dissection flap fenestration at the levelof proximal aortic arch, the Bentall procedure with onebutton re-implantation, and the left anterior descendingcoronary artery-left internal thoracic artery anastomosiswas done. After an uneventful postoperative period, thepatient was discharged with full recovery on the 10th day. ">
