Mycobacterial infections in heart transplant recipients

Mikobakteriyel enfeksiyonlar sıklıkla immünsüpresyonsonucunda meydana gelir ve genellikle morbiditeve mortalitenin nedenidir. Katı organ ve kök hücrenaklinden sonra mikobakteriyel enfeksiyonlarınepidemiyoloji ve klinik özellikleri iyi tanımlanmıştır.Ancak, klinik verilerin ender olması nedeniyle, kalpnakli sonrasında mikobakteriyel enfeksiyonlaraait özellikler henüz aydınlatılmamıştır. Kapsamlıbir literatür taraması, kalp nakli sonrasındamikobakteriyel enfeksiyonların genellikle nakildensonra geç dönemde ortaya çıktığını gösterdi. Kütanözve pulmoner enfeksiyonlar, en sık baskın patojenolarak Mycobacteria tuberculosis ile birlikte idi. Katıorgan nakil alıcılarının aksine, kalp nakli alıcılarındatüberküloz dışı mikobakteriyel enfeksiyonlar, belirlibir baskın suş olmaksızın, sporadikti. Kombinetedavinin monoterapiden daha etkili olduğu görüldü.Genel sağkalım oranı, %84.2 idi.

Kalp nakli alıcılarında mikobakteriyel enfeksiyonlar

Mycobacterial infections frequently develop as aconsequence of immunosuppression and are often acausative cause of morbidity and mortality. Epidemiologyand clinical characteristics of mycobacterial infectionsafter solid organ and stem cell transplantation have beenwell-described. However, due to the rarity of clinical data,the pertinent aspects of mycobacterial infection after hearttransplantation remain to be clarified. A comprehensiveliterature collection revealed mycobacterial infectionsafter heart transplantation usually developed late aftertransplantation. Cutaneous and pulmonary infectionswere the most common with Mycobacteria tuberculosisbeing the prevailing pathogen. Unlike in solid organtransplant recipients, non-tuberculous mycobacterialinfections in heart transplant recipients were sporadicwith no prevailing species. Combined drug therapyseemed to be more effective than monotherapy. Theoverall survival rate was 84.2%.
