ÜÇ BOYUTLU (3D) LAPAROSKOPİK KOLESİSTEKTOMİ; ISPARTA ŞEHİR HASTANESİ’NDEKİ BAŞLANGIÇ DENEYİMİMİZ
Laparoskopik cerrahi; cerrahi travmayı, hastanedekalış süresini, ameliyat sonrası ağrıyı azaltması gibiavantajlara sahiptir. Laparoskopik cerrahide gelişenteknolojiyle birlikte meydana gelen yenilikler laparoskopik prosedürlerin güvenliğini ve uygulanabilirliğiniartırmıştır. İki boyutlu (2D) laparoskopinin kısıtlılıklarıüç boyutlu (3D) laparoskopik sistemlerin gelişmesiyle azalmıştır. Geçmişte yapılan bazı çalışmalarda 3Dsistemlerin çok fazla avantaj sağlamadığı yönündeçıkan sonuçlar o dönemdeki ve günümüzdeki 3D sistemler arasında ciddi teknolojik farklılıklar nedeniyleson dönemlerde yapılan çalışmaların sonuçlarıylatam olarak karşılaştırılmamalıdır. 3D ve 2D laparoskopiyi karşılaştırarak yapılan araştırmalar komplike işlemlerde 3D sistemlerin işlem süresini ve hata oranınıazalttığı gösterilmiştir. Çalışmamızdaki vakalarda 3Dgörüntünün derinlik hissinin verdiği anatomiye hakimiyet ve geniş çalışma alanı ameliyatın tüm aşamalarında daha güvenli ve daha kararlı hareket imkanı sağladı. Maliyete bağlı nedenlerden dolayı 3D sistemlerinyaygınlaşamaması bu konuda yeterli çalışma yapılamasına engel olmaktadır. 3D sistemlerin etkinliğinindaha iyi değerlendirilmesi için uzun süreli ve genişvaka sayılı çalışmalara ihtiyaç vardır.
OUR THREE-DIMENSIONAL (3D) LAPAROSCOPIC CHOLECYSTECTOMY EXPERIENCE AT ISPARTA CITY HOSPITAL
Laparoscopic surgery has advantages ilke lesser surgical trauma, hospitalisation and postoperative pain. The innovations that come with the technological developments in laparoscopic surgery have increased the safety and feasibility of laparoscopic procedures. Limitations of two-dimensional (2D) laparoscopy have been reduced by the development of three-dimensional (3D) laparoscopic systems. The results that 3D systems do not offer many advantages in some previous studies should not be compared exactly with the results of recent studies due to serious technological differences between the old and current 3D systems. Studies comparing 3D and 2D laparoscopy have shown that 3D systems reduce the operation time and error rate in complicated procedures. The sense of full control and visibility of the anatomical images given by the 3D view and wide working space enabled the safer and more stable movement at all stages of the operation. The inability of 3D systems to spread due to financial reasons is hindering adequate work in this regard. For better evaluation of the effectiveness of 3D system long-term case studies needed.
