Kayseri bölgesinde bruselloz seroprevalansı: Dört yıllık değerlendirme

Bruselloz Brucella cinsi bakterilerin yol açtığı zoonotik bir hastalıktır. Tüm dünyada yaygın olarak görülmektedir. Hala ülkemizde önemli mortalite ve morbiditeye neden olan bir halk sağlığı sorunu olmaya devam etmektedir. Bu çalışmada bölgemizdeki bruselloz seroprevalansının araştırılması amaçlanmıştır. Bu çalışmada Ocak-2010 ve Aralık-2013 tarihleri arasında Erciyes Üniversitesi Tıp Fakültesi Hastaneleri poliklinik ve servislerden bruselloz ön tanısıyla gönderilip, çalışmaya alınan 21288 hastanın serum sonuçları retrospektif olarak değerlendirilmiştir. Bu serum örnekleri Wright Tüp Aglütinasyon ve Coombs Aglütinasyon testi ile çalışıldı. Çalışmada 1/10 üzeri aglütinasyon seropozitiflik olarak kabul edilirken, 1/160 ve üzeri titrede aglütinasyon görülmesi bruselloz teşhisi için referans değer olarak kabul edilmiştir. Bu çalışmanın sonuçlarına göre, Bruselloz ön tanısıyla laboratuvara gönderilen 21288 hasta serumunun 965 (%4.5)inde 1/10 ve üzerinde seropozitiflik saptanmış- tır. 1/160 ve üzeri titrasyonda pozitiflik oranı ise %1.2 (250/21288) olarak belirlenmiştir. Bu 250 pozitif örneğin yaş ortalaması 36.5±22.5 yıl ve kadın/erkek oranı ise 99/151 (0.65) olarak bulunmuştur. Dünyada ve ülkemizde bruselloz seroprevalansı coğrafi durum, gelişmişlik düzeyi ve sosyoekonomik duruma göre farklılıklar göstermektedir. Bruselloz önleme ve eradikasyon programlarının başarılı olması için özellikle sürveyans çalışmalarına önem verilmesi ve çok merkezli çalışmalar yapılması gerekmektedir.

Seroprevalence of brucellosis in Kayseri region: Four-years review

ABSTRACT Brucellosis caused by bacteria of the genus Brucella is a zoonotic global disease which is seen common world-wide. This disease still continues to be a public health problem which causes significant morbidity and mortality in our country. In this study, we aimed to investigate the seroprevalence of brucellosis in our region. In this study, conducted between January 2010 and December 2013 in Erciyes University Medical Faculty Hospitals with the prediagnosis of brucellosis sent from outpatient clinics and services, the serum results of 21288 patients were evaluated retrospectively. These serum samples were studied with Wright and Coombs tube agglutination tests. In this study, agglutination over 1/10 was considered as seropositivity, agglutination titer of 1/160 and above was accepted as a reference value for the diagnosis of brucellosis. According to the results of this study, 965 (4.5%) of 21288 patient sera sent to laboratory with pre- diagnosis of brucellosisfound 1/10 and above seropositivity. Titer of 1/160 and higher positive rate was detected as 1.2% (250/21288). Mean age and female/male ratio of these 250 positive samples were 36.5 ± 22.5 years and 99/151 (0.65) respectively. The seroprevalence of brucellosis in the world and in our country varies depending on geographical situation, development level and socioeconomic status. For success of brucellosis prevention and eradication programs, it is necessary to pay a special attention to surveillance and to perform multicenter studies.
