HEMATOLOJİK MALİGNİTELİ HASTALARDA KARBAPENEME DİRENÇLİ GRAM NEGATİF BAKTERİ KOLONİZASYONUNUN ARAŞTIRILMASI
İmmünsüprese hastaların yattığı servislerde karbapenem dirençli gram negatif bakteri (KDGNB)’nin kolonizasyonunu önlemek için risk faktörlerini belirlemek önemlidir. Bu çalışmanın amacı kemik iliği nakli (KİT) ünitesine yatan hastalarda KDGNB kolonizasyonu için risk faktörlerini belirlemek ve enfeksiyon gelişimi üzerine etkisini saptamaktı. Bu çalışma KİT üniteleri’nde yatan 18 yaşın üzerindeki hastalardan haftada bir kez alınan rektal sürüntü örnekleri kullanılarak yapıldı. KDGNB tanımlaması Mikrobiyoloji Laboratuvarı’nda yapıldı. Her hasta çalışmaya bir kez alındı. Toplam 125 hasta çalışmaya alındı. Hastaların yaş medyanı 49 yıl ve %56,8’i erkek idi. Hastaların 18 (%14,4)’inde rektal sürüntü kültürü örneğinde KDGNB kolonizasyonu saptandı. Çoklu lojistik regresyon analizinde, KDGNB kolonizasyonunu etkileyen faktörler dış merkezden nakil, öncesinde yoğun bakım ünitesi (YBÜ) yatışı, solunum yetmezliği ve bronkoskopi işlemi uygulanması olarak belirlendi. Bunların içinde dış merkez öyküsü en düşük etkili faktör olarak görülürken yatış öncesinde YBÜ yatışı olması kolonizasyon gelişimini 47,39 kat, solunum yetmezliği 63,21 kat, bronkoskopi işlemi yapılması ise 9,16 kat arttırmaktaydı. Ayrıca etkenler kolonize hastaların % 27,7’sinde sistemik bir enfeksiyona neden oldu. Erken ve uygun ampirik tedavinin başlanabilmesi için KDGNB kolonizasyon oranını ve kolonizasyon için risk faktörlerinin bilinmesi önemlidir. KİT ünitelerinde KDGNB kolonizasyonunun takibi, ekonomik olarak şartları uygun olan hastanelerde yararlı olabilir
Evaluation of the Patients with Hematological Malignity in Terms of the Colonization with Carbapenem Resistant Gram Negative Bacteria
In clinics staying immunocompromised patients, it is important to determine the risk factors for the colonization of carbapenem resistant gram negative bacteria (CRGNB). The aim of this study was to determine the risk factors of CRGNB colonization in hospitalized patients in bone marrow transplantation (BMT) unit and to find out the effect on infection development of the colonization. This study was performed on patients over 18 years, taken routinely rectal swab samples weekly, and hospitalized in BMT units. Identification of CRGNB was performed in the Microbiology Laboratory. Every patient was enrolled in the study once. A total of 125 patients were enrolled in the study. The median age of patients was 49 years and 56.8% of them were male. CRGNB colonization was detected in 18 patients (14.4%) from rectal swab sample. The factors affecting the colonization with CRGNB were defined as transfer from a hospital, previous intensive care unit (ICU) admission, respiratory failure and bronchoscopy procedure in multivariate logistic regression analysis. The transfer from a hospital was found out as the lowest effective factor, stay in ICU increased development of the colonization 47.39 times, respiratory failure 63.21 times, and bronchoscopy procedure 9.16 times. Moreover, 27.7% of colonized patients gave rise to a systemic infection. Knowing the frequency of CRGNB colonization and the risk factors of colonization is important for starting early and appropriate empiric treatment. Surveillance of CRGNB colonization in the BMT unit can be useful in hospitals having sufficient support.
