Alt üriner sistem yakınmaları ile başvuran hastalarda prostat kanseri erken tanısı ve hastaların yıllık takip protokolüne uyumu

Prostat kanseri erken tanısında 50 yaş üzerindeki erkeklerde yıllık prostat spesifik antijen (PSA) ve parmakla rektal muayene (PRM) uygulaması önerilmektedir. Bu çalışmada alt Üriner sistem yakınmaları ile başvuran hastalarda prostat kanseri erken tanısı ve benign prostat hiperplazisi tanısı ile tedavi edilen hastaların prostat kanseri açısından yıllık takip protokolüne uyumu araştırıldı. Bir yıl içerisinde toplam 779 hasta alt Üriner sistem yakınmaları ile üroloji polikiniğimize başvurdu. Tüm hastalarda PRM ve PSA ile prostat kanseri taraması yapıldı. Sonuçları normal olan tüm hastalar benign prostat hiperolazisine yönelik hangi tedavi alırlarsa alsınlar yıllık takip gerekliliği konusunda bilgilendirilerek takip protokolüne alındı. PSA 4 ng/ml üzeri ve/veya PRM sonucu anormal olan toplam 156 hastaya prostat biyopsisi uygulandı ve sonuçta alt Üriner sistem yakınmaları ile başvuran hastalarda %4 (30/779) oranında prostat kanseri saptandı. Klinik evrelendirme sonucunda 20 hastada metastatik, 10 hastada ise lokal prostat kanseri belirlendi. Öte yandan, prostatektomi yapılan 115 hastanın sadece 14'ünün (%12) 1. yılda bu amaçla kontrole geldiği görüldü. Uzun dönem (bir yıldan uzun) medikal tedavi önerilen hastaların (toplam 33) ise daha büyük oranda (14 hasta, %42) prostat kanseri taraması için geldikleri izlendi, îlk biyopsi sonucu benign olan hastaların bile (125 hasta) sadece 39'nun (%31,2) kontrole geldiği saptandı. Sonuç olarak bu çalışmada, alt Üriner sistem yakınmaları ile başvuran hastalarda PSA ve PRM kombinasyonu ile %4 oranında prostat kanseri saptanmıştır. Ancak hastaların çoğunluğunun küratif tedavi sınırlarının dışında olduğu görülmektedir. Öte yandan prostat kanseri takip protokolü (yıllık PSA+PRM) önerilen hastaların büyük çoğunluğu birinci yıldaki kontrollerine dahi gelmemişlerdir. Bu oran özellikle prostatektomi grubunda oldukça yüksektir. Dolayısıyla prostat kanseri taraması gündeme geldiğinde kamuoyuna yönelik bilgilendirme çalışmalarına gereksinim duyulmaktadır.

Early diagnosis of prostate cancer in patients with lower urinary tract symptoms and evaluation of compliance with the annual follow-up protocol

It is widely accepted that the combination of digital rectal examination (DRE) and prostate specific antigen (PSA) is the best method in the early detection of prostate cancer in terms of cost-effectiveness. In this study, early diagnosis of prostate cancer in a urology outpatient department was assessed. Additionally, compliance of the patients who received alternative treatments for benign prostate hyperplasia with the follow-up protocol for prostate cancer was investigated. Each patient over 50 year of age underwent DRE and PSA combination for prostate cancer screening. While patients with PSA >4 ng/ml and/or abnormal DRE underwent prostate biopsy, the others were included in an annual follow-up protocol whatever the treatment for benign prostate hyperplasia they received. Prostate cancer was detected in 4% of patients (30/779) who attended to the outpatient department with symptoms of prostatism after a total of 156 biopsies. Initial staging re-vealed clinically localised disease only in 10 and metastatic dise-ase in the remaining 20 patients. On the other hand, only 12% of patients who underwent prostatectomy for benign prostate hyperplasia came to follow-up visit at one year for prostate cancer screening. This ratio was 42% in the medical treatment group. Even in the biopsy group with the initial diagnosis of benign prostate hyperplasia, only 31% of the patients were under follow-up at one year. In conclusion, there are still problems in early diagnosis of prostate cancer based on PSA and DRE. Since, majority of the prostate cancer patients is beyond the boundaries for the definitive treatment. Additionally, compliance of the patients with the follow-up protocol for prostate cancer screening was significantly low. This situation was strictly remarkable in patients who underwent prostatectomy for benign prostate hyperplasia.
