Periferik Nöropati Tanısında Klinik Ön Tanı ve Elektrodiyagnostik Tanı Korelasyonu; Branşlar Arası Fark Var Mı?
Elektronöromiyografi (ENMG) klinik değerlendirmenin ardından şüphe duyulan nöromusküler bozukluk hakkında bilgi veren, uygun protokollerle yapıldığında hastalığın tanı ve tedavisinde yardımcı olan bir tanı yöntemidir. İyi bir klinik değerlendirme ve doğru endikasyon varlığında, daha yüksek oranda tutarlı sonuç elde edilir. Çalışmamızın amacı Nöroloji Kliniği ENMG Ünitesi’ndeki 3 yıllık elektronörofizyolojik çalışma istemlerindeki ön tanılar ile elektronörofizyolojik tanıların uyumluluğunu saptamak ve tetkiki isteyen branşlar arasında uyumluluk açısından fark olup olmadığını tespit etmekti. Şubat 2016-Şubat 2019 tarihleri arasında Gülhane Eğitim ve Araştırma Hastanesi Nöroloji Kliniği ENMG Ünitesi’nde periferik nöropati ön tanısı ile yapılmış 400 elektronörofizyolojik çalışma, ön tanı ile elektronörofizyolojik tanı uyumluluğu açısından retrospektif olarak incelendi. ENMG Ünitesi’nde yapılan elektronörofizyolojik çalışmaların %47.25’i normal bulundu. Klinik ön tanı ile elektronörofizyolojik tanı arasında en iyi uyumluluk %55 oran ile Nöroloji Kliniği’nde idi, bu kliniği %43’lük oran ile Fiziksel Tıp ve Rehabilitasyon (FTR) Kliniği takip etti. Çalışmamızda klinik ön tanı ile elektronörofizyolojik tanının uyum oranında istem yapan klinikler arasında istatistiksel olarak anlamlı fark olduğunu bulduk. Klinik ön tanı ile elektrofizyolojik tanının uyumlu olması hastanın ilgili uzmanlık alanları tarafından iyi klinik değerlendirilmesinin yapılıp elektrodiyagnostik çalışmaların doğru endikasyonda istenmesine bağlıdır.
The Correlation Between Clinical Referral Diagnosis Versus Electrodiagnostic Diagnosis for Peripheric Neuropathy; Is There Any Difference Between Different Departments?
Electroneuromyography (ENMG) is a diagnostic method that helps to diagnose and treat the disease when it is performed with appropriate protocols following clinical evaluation, providing information about suspected neuromuscular disorder. In the presence of good clinical evaluation and correct indication, a more consistent result is obtained. The aim of our study was to determine the compatibility between the preliminary diagnosis in the request forms and the electroneurophysiological diagnosis in 3-year electroneurophysiological studies in the ENMG Unit of Neurology Department and to determine if there was any difference in terms of compatibility among the branches which request the examination. A total of 400 electroneurophysiological studies with preliminary diagnosis of peripheral neuropathy were assessed retrospectively between February 2016-February 2019 in ENMG Unit of Neurology Department of Gulhane Training and Research Hospital for the consistency of clinical referral diagnosis and electroneurophysiological diagnosis. Results of the electroneurophysiological studies performed in the ENMG Unit, 47.25% were normal. The best consistency of clinical referral diagnosis and electroneurophysiological diagnosis was in the Neurology Department with a rate of 55%. Physical Medicine Rehabilitation (PMR) Department followed it with 43%. In our study, we found that there was a statistically significant difference in the rate of compatibility of the clinical preliminary diagnosis and the electroneurophysiological diagnosis among clinics. Compatibility between the clinical preliminary diagnosis and electrophysiological diagnosis depends on the good clinical evaluation of the patient by the relevant fields of expertise and the demand of the electroneurophysiological studies in the right indication.
