Lidokain Toksisitesinin Lipid Emülsiyonu ile Başarılı Tedavisi

Bu bildiride, port katater takılması için yüksek doz ve volümde lidokain infiltrasyonuuygulaması sonrasında ani toksisite bulguları gösteren 28 yaşındaki ASA II kadınhasta sunulmaktadır. Literatürdeki olgulardan farklı olarak, lidokain toksisitesi santralveya periferik blok tekniği uygulamadan gelişti. Sistemik toksisite nedeniyle bilinci kapananve ventriküler taşikardi gelişen hasta olası bir kardiyak arrest meydana gelmedenacil lipid emulsiyonu başlanmasıyla başarıyla tedavi edildi. Lipid tedavisi sonrasında bilinciaçılan hasta nefes almakta güclük çektiğini bildirdi. Çekilen akciğer grafisi sonrasındakateter takma işlemi sırasında komplikasyon geliştiği fark edildi ve hastaya pnömotorakstanısı konuldu.Lokal anestezik toksisitesi gelişen olguların resüsitasyonunda lipid tedavisinin hayatkurtarıcı öneminin olduğu ve ameliyathane, yoğun bakım ve acil serviste resüsitasyongereçlerinin yanında lipid infüzyonu için kullanılacak preparatların hazır bulundurulmasının gerektiği kanısındayız.
