ERİŞKİN İZOLE MONTEGGİA KIRIKLI ÇIKIKLARI CERRAHİ TEDAVİSİ VE FONKSİYONEL SONUÇLARI

Giriş: Monteggia kırıklı çıkığı dirsek eklemini de ilgilendiren kompleks bir yaralanmadır. Bu çalışmamızda Erişkin İzole Monteggia Kırıklı Çıkığı tanısıyla takip ve tedavi ettiğimiz hastaların fonksiyonel sonuçlarını değerlendirmeyi amaçladık. Gereç ve Yöntem: Hastalar retrospektif olarak değerlendirildi. Kriterlere uyan 8 hastaya ulaşılarak değerlendirildi: Yaralanmalar Bado ve Jüpiter sınıflamasına göre sınıflandırıldı. Hastalar Broberg Morrey Functıonal Rating Index, ASES, Mayo Elbow Performans Skorlaması ve DASH ile değerlendirildi. Fonksiyonların değerlendirilmesinde dirsek eklem hareket açıklıkları gonyometre ile ölçüldü ve karşı dirsek ile karşılaştırıldı. Bulgular: Hastaların 3’ünde Bado Tip 1, 5’inde Bado Tip 2 Monteggia kırıklı çıkığı mevcuttu. Hastaların 1’i kötü sonuç olarak değerlendirilirken; 5 hastanın sonuçları iyi, 2 hastanın ise mükemmel olarak değerlendirildi Hastaların tümünde ulna’ya açık redüksiyon internal fiksasyon uygulanırken radius başı çıkığı kapalı redükte edildi. Hastaların hiçbirisinde ameliyat öncesi ve sonrasında nöropraksi ve enfeksiyon izlenmedi. 2 hastada dirsek hareketleri karşı tarafa göre kısıtlı idi. Sonuç: Erişkin Monteggia Kırıklı Çıkığı olan hastalarda erken cerrahi girişimle tedavi ile başarılı sonuçlar elde edilebilmektedir. Bu hastalarda ulna kırığının uygun redüksiyon ve fiksasyonu tedavi sonuçlarını etkilemektedir.

SURGICAL TREATMENT AND THE FUNCTIONAL RESULTS OF ADULT ISOLATED MONTEGGİA FRACTURE-DISLOCATIONS

Introduction: Monteggia fracture-dislocation is a complex injury which comprises elbow joint. In this study we aimed to evalute the functional results of th patients who had surgical treatment because of Monteggia fracturedislocation. Material and Method: In our study we evaluated 8 patients who adhered to the creteria, retrospectively. Injuries classified by Bado and Jupiter Classifications. At the last visit of the patients we evaluated their Broberg Morrey Functıonal Rating Index , American Shoulder and Elbow Society Score (ASES) , Mayo Elbow Score and Disabilities of The Arm Shoulder and Hand Scores (DASH). The patients’ forearm pronation/supination and elbow flexion and extensions were assesed. Results: 3 of the patients were Bado type 1 and 5 of them were Bado type 2. 5 of the patients were evaluated as good , 2 of them were excellent and 1 of them was poor outcome. We have treated all the patients with open reduction and internal fixation for ulnar fractures and closed reduction for the radıal head dislocations. At preoperation or postoperation none of the patients had neuropraxia or infection. 6 patients’ elbow Rom were the same with the non-operated elbow. Conclusıon: It is a surgical urgency in which anatomic reduction and stabilization with the correct implants are essential for good outcomes.
