Gibboziteye neden olan ve tedavi altında farklı seviyede reaktivasyon gösteren Pott Hastalığı
Amaç: Pott Hastalığı; Mycobacterium tuberculosis'in neden olduğu vertebral osteomyelit olarak tanımlanmaktadır. Bu yazıda gibboziteye neden olan ve tedavi altında farklı seviyede reaktive olan Pott Hastalığı bulunan bir olgu sunulmaktadır. Olgu sunumu: Otuz iki yaşında bayan hasta sırt ağrısı şikayeti ile kliniğimize başvurdu. Hastanın torakal manyetik rezonans görüntülemesinde (MRG) Th 6-7 vertebra osteomyeliti tespit edildi. Hasta opere edildi. Sol anterolateral yaklaşımla Th5-6 abse drenajı, korpektomi ve anterior stabilizasyon+füzyon uygulandı. Hastanın ameliyat sonrasında sırt ağrısı şikayeti düzeldi. Histopatolojik ve mikrobiyolojik inceleme sonuçları tüberküloz olarak rapor edildi. Anti tüberküloz tedavisi başlandı. İlk ameliyattan 3 ay sonra hasta bel ağrısı ve yürümede güçlük şikayetleri ile tekrar kliniğimize başvurdu. Hastanın bu sürede önerilen anti tüberküloz ilaçlarını düzenli bir şekilde aldığı öğrenildi. Çekilen MRG’de lomber vertebra osteomyeliti tespit edildi. Hasta tekrar opere edilerek anterior yaklaşımla L2 vertebral abse drenajı, korpektomi, diskektomi uygulanıp anterior stabilizasyon ve füzyon uygulandı. Sonuç: Son yıllarda tüm dünyada ve ülkemizde tüberküloz olgularında artış kaydedilmektedir. Omurga tüberkülozu iskelet sistemi tüberkülozları içerisinde en sık görülen tip olup hastalık günlük nöroşirurji pratiğinde tekrar gündeme gelmeye başlamıştır.
Pott's disease which caused gibbosity and reactiveted under treatment
Objective: Pott's disease is defined as vertebral osteomyelitis caused by Mycobacterium tuberculosis. In this report we present a case of Pott's disease which caused gibbosity and reactivated under treatment. Case Report: Thirty-two years old female patient was admitted to our clinic with complaints of back pain. The patient's thoracic magnetic resonance imaging (MRI) was showed Th 6-7 vertebral osteomyelitis. The patient was operated with left anterolateral approach and abscess drainage, corpectomy and anterior stabilisation+fusion was performed. The patient’s back pain was resolved after surgery. Histopathological and microbiological results were reported as tuberculosis. Anti-tuberculosis treatment was started. Three months later from the operation the patient readmitted to us with complaints of low back pain and difficulty in walking. In this period she was used her antituberculosis treatment regularly. In MRI lumbar vertebral osteomyelitis was determinated. The patient was reoperated with anterolateral approach and vertebral abscess drainage, discectomy, corpectomy and anterior stabilisation and fusion was performed. Conclusion: In recent years, an increase in tuberculosis cases is recorded. Spinal tuberculosis is the most common type of disease within the skeletal system and started come back important in daily practice of neurosurgery.
