Geniş ve komplike dekübit ülserlerinin kalça dezartikülasyonu ve tam uyluk flebi ile tedavisi

Amaç: Geniş ve komplike dekübit ülserleri eski bir tedavi yöntemi olan “tam uyluk” flebi ile tedavi edilebilir. Flep kalça dezartikülasyonu ile hazırlanır ve defekti kapamak için kullanılabilir. Bu yazıda olgumuz nedeni ile “tam uyluk” flebini yeniden hatırlatmayı amaçladık. Olgu sunumu: Doğumdan bu yana her iki bacakta spastisitesi olan 35 yaşında erkek hasta geniş sakral ve trokanterik dekübit ülseriyle kliniğimize başvurdu. Hastanın başka bir klinikte sakral dekübit ülseri nedeniyle bilateral gluteus maksimus V-Y ilerletme kas deri flebi kullanılarak ameliyat edildiği, ancak trokanterik dekübit ülserine yönelik bir operasyon geçirmediği öğrenildi. Spastisitesine bağlı yürüyememesi ve ayaklarını kullanamaması ve başka iyi bir rekonstrüksiyon alternatifi olmaması nedeniyle kalçadan dezartikülasyon yapılmasına karar verildi. Postoperatif dönemde komplikasyon görülmedi. Sonuç: Lokal ve serbest flep alternatifi olmayan geniş dekübit ülserleri “tam uyluk” flebi ile tedavi edilebilir.

Treatment of large and comlex pressure sores with hip disarticulation and total thigh flap

Objective: Large and complex decubitus ulcers can be treated by using total thigh flap which is an old treatment method. The flap is obtained after hip disarticulation and can be used for coverage of defects. Aim of this paper is to remember again total thigh flap with our case. Case Report: 35 year old male patient who had bilateral leg spasticity since his birth has applied to our clinic due to large sacral and trochanteric decubitus ulcers. Patient was operated by another clinic by using bilateral gluteus maximus musculocutaneous V-Y advancement flaps for sacral decubitus ulcer, but no operation for trochanteric decubitus ulcer. It was decided to disarticulate of right hip because of his spasticity and was not able to walk and use his legs and lack of another reconstructive options. There were no complications at postoperatively. Conclusion: Large decubitus ulcers with no local and free flap alternatives can be covered with total thigh flap.
