We repon a case with divided piriformis muscle together with the sciatic nerve dividing high and the common fibular nerve passing between the two divisions of the piriformis muscle in both gluteal regions. Regarding the clinical syndrome of the piriformis muscle; this should be taken into consideration by clinicians.
