Nadir Görülen Bir Suicid Yöntemi, Elma Çekirdekleri: Bir Olgu Sunumu

Amigdalinin dahil siyanojenik glikozitler doğal olarak oluşan bitkisel toksinlerdir. Elma çekirdekleri potansiyel olarak toksik bir bileşik olan amigdalin içerir. Oral yolla alınan amigdalin ince bağırsakta bulunan beta glukuronidaz ile siyanüre hidrolize edilir ve siyanür zehirlenmesine yol açar. Nadiren kullanılan ve karmaşık yöntemleri içeren suicid girişimleri özellikle önemlidir ve oral yolla siyanüre dönüşebilecek toksinlerin alımı da bu kapsamda düşünülebilir. Suicid girişimi ile başvuran olgularda kişilik bozukluğu sıklıkla teşhis edilmektedir ve Borderline Kişilik Bozukluğu toplum içerisinde suicid girişimleri açısından riskin en yüksek olduğu tanılardan biridir. Bu olgu sunumunda elma çekirdekleri ile suicid girişiminde bulunan ve borderline kişilik bozukluğu tanısı olan bir hastada siyanür toksisitesinin belirti ve bulgularının erken dönemde tanınmasının ve tedavi edilmesinin başarılı bir klinik sonuç için hayati önem taşıdığı gösterilmeye çalışılmıştır. Özellikle kişilik bozukluğu olan hastalarda nadir görülen suicid yöntemlerine ve olası risk faktörlerine yeniden dikkat çekmek amaçlanmıştır.

A Rare Suicid Method, Apple Kernels: A Case Report

Cyanogenic glycosides including amygdaline are naturally occurring herbal toxins. Apple seeds contain amygdalin, a potentially toxic compound. Orally amygdalin is hydrolyzed to cyanide with beta-glucuronidase in the small intestine and leads to cyanide poisoning. Suicidal attempts which are rarely used and involve complex methods are particularly important and oral intake of toxins that can turn into cyanide can also be considered in this context. Personality disorders are frequently diagnosed in cases presenting with suicid attempt and borderline personality disorder is one of the highest risk in terms of suicid attempt in the community. In this case report, a patient who attempted suicid with apple kernels and considered borderline personality disorder is presented. It is aimed to demonstrate that the early recognition and treatment of signs and symptoms of cyanide toxicity is vital for a successful clinical outcome and to draw attention to rare suicidal methods and possible risk factors, especially in patients with personality disorders.
