Peyronie hastalığı tunika albuginea'nın inflamatuar bir hastalığıdır. Belirgin penil deformite gelişen hastalarda ise plağın çıkarılması ve greft uygulaması seçilebilecek cerrahi tedavi seçeneğidir. Bu çalışmanın amacı peyronie plak eksizyonu ve tunika vaginalis greft (TVG) uyguladığımız olgulardaki takip sonuçlarımızı değerlendirmektir. 1996 ile 1999 yılları arasında en az 1 yıldır plak öyküsü olan ve 45 dereceden fazla eğrilik saptadığımız 7 Peyronie hastası çalışmaya alındı. Erektil disfonksiyon (ED) saptanmayan olguların yaş ortalaması 51±6.5 (37-58), hastalığın başlangıcı ile cerrahi tedavi arasındaki geçen ortalama süre 32.3±13.7 (12-64) aydı. Subkoronal insizyonel yaklaşımla plak eksizyonunu takiben tunika vaginalis grefti ile korporal defekt kapatıldı. 7 hastada ortalama 1.8 cm2 alanında toplam 11 greft kullanıldı. Olguların tümünde operasyon ile eğrilik düzeldi. Ortalama 23±7.9 aylık (8-36) izlemde potens korunurken greft uygulanan alanda şişme ve penisde kısalma yakınması olmadı. Bir olguda post-operatif 9. ayda 30 derecelik eğrilik saptandı. Tedavi sonucuna göre hasta memnuniyeti % 100 idi. Peyronie plak eksizyonunu takiben defektin hastanın kenditunika vaginalisi ile kapatılması basit, masrafsız ve tatminkar sonuçlar aldığımız güvenli bir yöntemdir. Elde edilen bu ön sonuçların daha büyük serilerde onaylanması gerekmektedir.

Peyronie's disease is an inflammatory disorder of the tunica albuginea of the penis. Excision of the plaque and grafting is the another treatment of choice for the correction of penil deformity. The aim of this study was to evaluate the outcomes of tunica vaginalis graft repair after Peyronie's plaque excision. From 1996to 1999, 7 patients with at least 1 year history of Peyronie's disease with penil curvature greater than 45 degrees were evaluated. Their mean age as 51±6.5 (range: 37-58) years and the mean duration of disease was 32.3±13.7 (range: 12-64) months. All of them had normal sexual potency. Autolog tunica vaginalis graft was used to close the corporotomy defect after plaque excision, via a subcoronal incisional approach. Eleven graft were used in 7 patients with average 1.8 cm² surface area. Surgical correction of the curvature was achieved in all patients. At mean follow-up of 23±7.9 (range: 8-36) months, all patients were potent without requiring medical therapy. No complaints of penile shortening and bulging at the graft site have been noted. One patient reported a 30-degree curvature in operated site after 9 months postoperatively. Overall patient satisfaction with the procedure was 100 %. The use of tunica vaginalis grafts for the coverage of corporotomy defects after Peyronie's plaque excision is a simple, safe method with no additional expense and allows for satisfactory clinical results. These preliminary results need to be confirmed in larger series.
