Caput Longum Musculi Bicipitis Brachii'nin yapışma yeri ve omuz stabilitesinin sağlanmasındaki önemi

Amaç: Musculus biceps brachii kolun on bolumünde bulunan iki baslı fusiform bir kastir. Caput breve olarak adlandırılan kisa başı processus coracoideus'dan, caput longum olarak adlandırılan uzun başı ise tuberculum supraglenoidale'den baslar. Tuberculum supraglenoidale ile birlikte cavitas glenoidalis'in posterior kenarında labrum glenoidale ile devam eden caput longum musculi bicipitis brachii'nin (CLMBB) tendonu omuz ekleminin stabilitesinin saglanmasmda etkin rol oynamaktadir. Omuz ekleminde anterior subluksasyon, dislokasyonlar yada yasli hastalarda goriilen dejeneratif lezyonlar labrum glenoidale'de yirtiklara neden olabilmektedir. CLMBB'nin tendonu'nun labrum glenoidale'ye yapisma yerindeki farkkliklar omuz stabilitesi ve labrum glenoidale'deki yirtiklann yerlesimi ile iliskilidir. Calismamizda CLMBB'nin tendonunun yapisma yerlerindeki farkliliklar belirlenerek, bu farklikklann omuz eklem stabilitesi iizerine etkileri vurgulanmaya calisilmistir. Gereg ve yontem: Bu arastmna DEUTF Anatomi Anabilim dalinda formaldehid ile fiske ediknis 21 (11 sag, 10 sol) adet yetiskin erkek omuz eklemi kullanilarak yapil-mistir. Bulgular: Qalismamizda CLMBB'nin tendonlarinin yapisma yerleri degerlendirildi. %52,38 olguda CLMBB cavitas glenoidale'nin posterosuperior'una, %47,62 olguda ise cavitas glenoidale'nin superior'una yapismaktadir. Posterosuperior'a yapisah olguların %90,90'inda CLMBB'nin labrum glenoidale'ye, %9,10'unda ise hem labrum glenoidale hem'de tuberculum supra glenoidale'ye yapistigi gozlendi. Cavitas glenoidale'nin superior'una yapisan olgulaon %50'sinde CLMBB'nin labrum gienoidale'ye, %30'unda hem labrum glenoidale hem de tuberculum supraglenoidale'ye, %20 sinde ise sadece tuberculum supraglenoidale'ye yapistigi gozlendi. Sonu§: CLMBB'nin tendonu'nun cavitas glenoidalis'e yapisma yeri superior labrum avulsiyonlarinin ve ona bağlı omuz instabilitesinin tedavisinde rol oynamaktadir. Tendonun labruma veya kemige fixe olmasi bu bolge cerrahisinde tendon ve labrumun instabiliteyi gidermek ifin reinsersiyonuna yon verebilecegini düşünmekteyiz.

The attachment of long head of the biceps Brachii and its relationship with the stabilization of the shoulder joint

Objective: The short head of the biceps muscle arises from the apex of the corocoid process as the long head arises from the supraglenoid tubercle. The long head of biceps brachii muscle (BBM) continuous with the posterior part of the gle-noid labrum and plays an important role in the stabilization of the shoulder joint. It is well known that anterior subluxation, dislocation or degenerative lesions in elder.patients are the major cause of tears of the glenoid labrum. It is well known tha the differences of insertion of the long head of BBM tendon to the glenoid labrun are related with the shoulder instability and labral avulsions of the glenoid. Material and method: This study was performed at the Dokuz Eylul Universit Department of Anatomy. The specimens consisted of formol fixed 21 shoulde joints of adult human males (11 right,10 left). Results: We evaluated the attachment site of the tendon of long head of BBM an< its relationship to the the glenoid labrum. In about 52.38% of the specimens, we ob served that the tendon was attached to the posterosuperior margin of the glenoii cavity, whereas in 47.62% the attachement site was the apex of the glenoid cavit) In specimens by which the tendon was attached to the posterosuperior margin, th attachement site was the labrum glenoidale (90.9%) and both the labrum and th supraglenoidal tubercle of the glenoid cavity (9.10%). However, by specimens whicl attached to the cavitas glenoidale superior, the major attachement sites were th labrum glenoidale (50%), the labrum and tuberculum supraglenoidale (30%) an tuberculum supraglenoidale (20%), respectively. Conclusion: The insertion of long head of BBM to the glenoid rim plays a majo role in the shoulder stability. This study provides an important knowledge about th surgical anatomy of this area.
