Kemik metastazı yapmış meme kanserli hastaların tedavi sonuçları

Eylül 2001-Aralık 2003 tarihleri arasında Dicle Üniversitesi Tıp Fakültesi Radyasyon Onkolojisi’ne kemik metastazı yapmış meme kanseri tanısıyla başvuran 26 (%84) kadın, 5 (%16) erkek olmak üzere toplam 31 hasta değerlendirildi. Medyan yaş 43 yıl (23-79) olarak bulundu. Histopatolojik alt tiplerine göre invaziv duktal karsinom %88 (27), tubulolobüler karsinom %6 (2), inflamatuar meme kanseri %6 (2) bulundu. Hastaların %39’unda (12 hasta) vertebra metastazı, %26’sında (8 hasta) vertebra dışı kemik metastazı varken, %35’inde (11 hasta) ise hem vertebra hem de vertebra dışı kemik metastazı vardı. Hastaların ikisi radyoterapiyi (RT) kabul etmedi. Onbeş hastaya günlük 300 cGy dozlardan toplam 3000 cGy uygulandı, 6 hastaya günlük 400 cGy dozlardan toplam 2000 cGy uygulandı. Sekiz hastaya ise tek doz 800 cGy radyoterapi uygulandı. RT verilen 29 hastanın 18’inde (%62) tam palyasyon, 11’inde (%38) kısmi palyasyon sağlandı. Hastalarda Grade I-II cilt toksisite dışında önemli bir toksisiteye rastlanmadı. Sonuç olarak her üç grupta da palyasyon açısından benzer sonuçlar elde edilmiştir.

Radiotherapy results of breast cancer patietns with metastatic bone disease

Breast cancer patients with bone metastasis who had admitted to Dicle University Department of Radiation Oncology for palliative radiation therapy between September 2001 and December 2003 were evaluated. There were 31 patients (26 female, 5 male). Median age was 43 years (range 23- 79). Histopathological subtypes were infiltrating ductal carcinoma (88%), tubulolobuler carcinoma (6%) and inflammatory carcinoma (6%). Loci of bone metastasis were vertebra only in twelve patients (39%), non-vertebral bones only in 8 patients (26%) and both vertebral and nonvertebral bones in 11 patients (35%). Two patients had refused radiotherapy. Radiation doses were 3000 cGy with 10 fractions in 15 patients, 2000 cGy with 5 fractions in 6 patients and 800 cGy single fraction in eight patients. Complete palliation of pain was achieved in 18 patients (62%) and partial palliation was achieved in 11 patients (38%). Treatment related toxicity was grade I-II dermatitis. In conclusion; same respons rates in terms of palliation can be achieved in the three radiotherapy schedules.
