Basit ve kompleks dirsek çıkıklarının tedavi sonuçlarının değerlendirilmesi

Amaç. Bu çalışmada basit ve kompleks akut dirsek çıkıklı olguların tedavi sonuçlarınınincelenmesi amaçlandı. Yöntem. Akut dirsek çıkıklı 28 olgu çalışmaya alındı. Olguların ortalamayaşı 30,2 idi. Tüm olgular acil serviste teşhis konulmasını takiben çıkıkları redükte edilerek çıkığınbasit ve kompleks olmasına göre tedavisi planlandı. Olguların takibinde klinik olarak Mayo dirsekperformans skoru kullanıldı. Ayrıca tüm olgular radyolojik olarak heterotropik ossifikasyon,dejeneratif artirit olup olmamasına göre değerlendirildi. Bulgular. Yaralanma mekanizması 26hastada yüksekten düşme, 2 hastada ise araç dışı trafik kazasıydı. Olgular 17si posterior, 10uposterolateral ve 1 olguda da posteromedial çıkık idi. Olguların %35,7si izole basit çıkık, %64,3üise ek patolojiler olan kompleks dirsek çıkıkları idi. 4 olguda geç dönemde travma sonrası artritbulguları, 9 olguda geç dönem ektopik kemik gelişimi saptandı. Klinik olarak hastalarımızın%82,1i mükemmel sonuç, %18i iyi sonuç aldı. Sonuç. Basit dirsek çıkıklarında prognoz iyi iken,kompleks dirsek çıkıklarında eklem sertliği, heterotrofik ossifikasyon ve travma sonrası artrit gibikomplikasyonlar ortaya çıkmaktadır. Radius başı kırıkları mümkün olduğunca redükte edilerekdirsek stabilitesi sağlanmalı ve radius başı rezeksiyonu yapılmamalıdır. Redüksiyon sonrası dirsekimmobilizasyonu üç haftayı geçmemeli, aşırı germe egzersizleri verilmemelidir.

Evaluation of treatment results in acute simple and complex elbow dislocations

Aim. We aimed to investigate the treatment results of acute simple and complex elbowdislocations in our study. Methods. Twenty eight cases with acute elbow dislocation wereinvolved the study. The average age was 30.2. After reduction, the treatment was plannedaccording to the type of dislocation which is either simple or complex in the emergencydepartment. Mayo elbow performance score was used for the follow-up of cases. Radiologicalevaluation was performed to all cases for detection of heterotrophic ossification and degerativearthritis. Results. The mechanism of injury was fall from a high building in 26 patients, and trafficaccidents in two patients. There were 17 posterior dislocations, 10 posterolateral dislocations, and1 posteromedial dislocation. 35.7% of cases were isolated simple dislocations whereas 64.3% ofcases were complex elbow dislocations with additional pathology. We found degenerative arthritisin four cases as well as ectopic bone formation in nine cases. 82.2% of patients were rated asexcellent and 18% of patients were rated as good clinically after treatment. Conclusion. Simpleelbow dislocations have good prognosis, but complex elbow dislocations have complications suchas elbow stiffness, heterotopic ossification and posttraumatic arthritis. Radial head fractures shouldbe reduced to restore elbow stability whenever it is possible and radial head resection should notbe performed. After reduction elbow immobilization should not be more than three weeks andexcessive streching exercises should be avoided.
