Çocuklarda sürekli ayaktan periton diyalizinin sol ventrikül sistolik fonksiyonlarına etkisi
AMAÇ:Çalışmamıza son dönem böbrek yetmezlikli çocuklarda sürekli ayaktan periton diyalizi (SAPD)'nin sol ventrikül fonksiyonları üzerine etkisini araştırmak amacıyla yaşları 7-14 arasında 16 hasta alındı. YÖNTEM: Hastalara SAPD programı başlangıcında (sıfır) ve 3, 6, 9, 12. aylarda ekokardiyografik inceleme yapıldı. M-mode ekokardiyografi ile sol ventrikül sistol sonu ve diastol sonu çapları, interventriküler septum ve sol ventrikül arka duvarı kalınlıkları ölçüldü, sol ventrikül ejeksiyon fraksiyonu, fraksiyonel kısalma oranı ve sol ventrikül kütle indeksi hesaplandı. SONUÇLAR:Çalışma sonunda hiçbir parametrede çalışma başlangıcına göre istatistiksel olarak anlamlı bir değişiklik olmadığı görüldü. YORUM: Son dönem böbrek yetmezliği olan çocuk hastalarda SAPD'nin sol ventrikül sistolik fonksiyonlarını korumada kısa ve orta vadede etkili olabileceği ancak uzun dönemdeki etkilerinin belirlenebilmesi amacıyla daha geniş bir hasta grubunda uzun süreli çalışmaya gereksinim olduğu sonucuna varıldı.
The effect of continuous ambulatory peritoneal dialysis on left ventricular systolic functions in children
PURPOSE: The purpose of our study was to evaluate the effect of continuous ambulatory peritoneal dialysis (CARD) on left ventricular dimensions and systolic functions. METHODS: Sixteen children with end-stage renal failure were evaluated by echocardiography at the beginning of CARD program and then in the 3rd, 6th, 9th and 12th months. Left ventricular end-systolic and end-diastolic diameters, thickness of the interventricular septum and left ventricle posterior wall were measured with M-mode echocardiography and ejection fraction, fractional shortening of left ventricle and left ventricular mass index were calculated. RESULTS: There were no statistically significant changes in all parameters at the end of the study (12 months). CONCLUSION: In conclusion, according to our mid-term results, CARD seems to be effective in preserving left ventricular mass and systolic functions in children with end-stage renal failure. Further long-term studies with a larger patient group are required to determine long term effects of CARD on cardiac functions.
