Esansiyel hipertansiyonda perindoprilin arteriyel nabiz dalga hızı ve arteriyel nabız dalga hızı ilerleme zamanına etkileri

Nabız dalga hızı (NDH), periferik arter dalga formlarından yararlanılarak geniş arterlerin elastisitesini değerlendirmede kullanılan bir tekniktir. Bu çalışmada perindoprilin primer hipertansif olgularda arteriyel duvar sertliğinin bir indeksi olan NDH ile nabız dalga hızı ilerleme zamanına (NDHİZ) etkisi incelenmiştir. Çalışmaya primer hipertansiyon tanısı almış 24 (17 kadın, 7 erkek) hasta dahil edildi. Hastalara perindopril 4 - 8 mg tedavisi başlandı; tedavi 10 haftaya tamamlandı. NDH, karotis ve femoral arterler kullanılarak, online nabız dalga kaydına ve NDH'nın otomatik hesaplanmasına imkan veren Complior Colson cihazı (Createch Industrie, Fransa) ile hesaplanmıştır. On haftalık perindopril tedavisini takiben sistolik kan basıncı, diyastolik kan basıncı ve NDH değerlerinde istatistiksel olarak anlamlı azalma olurken; NDHİZ'nda anlamlı uzama saptanmıştır (sırası ile p ≤ 0.001, < 0.001, 0.003, 0.01). Tedavi öncesi ve sonrasında kalp hızında anlamlı değişiklik olmamıştır (p = 0.50). Sonuç olarak; esansiyel hipertansiyonlu hastalarda perindopril tedavisi ile kan basıncı ve karotis-femoral NDH değerlerinde azalma, NDHİZ'da uzama tesbit edilmiştir.

Effects of perindopril on the arterial pulse wave velocity and arterial pulse wave propagation time in essential hypertension

Pulse wave velocity (PWV) is a tecnique in which large artery elasticity is estimated from analysis of the peripheral arterial waveform. The aim of the present study was to investigate the effects of perindopril on PWV and pulse wave propagation time (PWPT) in patients with essential hypertension. Perindopril (4 - 8 mg) was given to 24 patients (17 women, 7 men) with essential hypertension for a period of 10 weeks. The carotid-femoral PWV was assessed by Complior Colson device (Createch Industrie, France). Although systolic blood pressure, diastolic blood pressure andcarotid-femoral PWV were decreased, PWPT level was increased in 10 weeks after oral perindopril administration(p ≤ 0.001, < 0.001, 0.003, 0.01, respectively). There was no significant differences at the heart rate (p = 0.50). Although blood pressure andcarotid-femoral PWV levels were decreased, PWPT level was increased after oral perindopril administration.
