Akut herpetik nevraljide gabapentin ve gabapentin - tramadol kombinasyonunun karşılaştırılması

Amaç: Herpes zostere bağlı akut ağrının, postherpetik nevraljide (PHN) bir risk faktör olduğu düşünülmektedir. Antikonvülzanların, antidepresanların ve opioidlerin postherpetik nevraljide etkin olduğu bildirilmiştir. Ancak, bu ilaçların akut herpes zosterde etkinliği ve PHN riskini azaltıp azaltmayacağı bilinmemektedir. Bu çalışmada gabapentin ile gabapentin tramadol kombinasyonunun herpes zoster enfeksiyonunun akut döneminde ağrı kontrolünündeki etkinliği ve postherpetik nevralji riski sınandı.Yöntem: AHZ ağrısı nedeni ile hastalığın ilk 4 haftasında antiviral tedavi görmüş olarak başvuran 39 hasta iki gruba ayrıldı. Grup G hastalarına gabapentin 300-900 mg/gün üç gün içinde titre edilerek başlandı ve gereğinde en fazla 3600 mg/gün olacak şekilde tedaviye devam edildi. Grup GT’ye gabapentin ile eş zamanlı olarak tramadol 50-150 mg/gün dozlarda titre edilerek verildi ve gereğinde en fazla 400 mg/gün olacak şekilde devam edildi. Grup GT’de VAS

Comparison of gabapentine and gabapentine - tramadol combination in acute herpetic neuralgia

Objectives: Acute herpes zoster associated pain is an important risk factor for postherpetic neuralgia (PHN). Anticonvulsants, antidepressants and opioids are shown to be effective in prevention of PHN. But, the effects of these drugs in acute herpetic pain and in the prevention of PHN are not known. In this study, the efficacy of gabapentine and gabapentine tramadol combination was compared in acute herpetic pain and in the prevention of PHN. Methods: 39 patients with acute herpes zoster pain (during 4th week of the disease) taken antiviral treatments were randomized to two groups. In group G, gabapentine was started as 300-900 mg/day gradually titrated in three days and continued on maximum 3600 mg/day in case.
