AKUT KORONER SENDROM VE HAFİF SOL VENTRİKÜL YETMEZLİĞİ İLE BAŞVURAN HASTADA SOL SİNÜS VALSALVADAN KÖKEN ALAN SAĞ KORONER ARTER ANOMALİSİ
Koroner arter anomalileri, neden olabildikleri göğüs ağrısı, nefes darlığı, miyokard iskemisi, kalp yetmezliği, ventriküler aritmi ve ani kardiyak ölüm nedeniyle ayırıcı tanıda dikkate alınması ge- reken klinik durumlardır. Karşı sinüs valsalvadan köken alan sağ koroner arter anomalisi oldukça nadir görülen, koroner anjiyografi sırasında standart kateterle ve manipülasyonla sağ koroner os- tiyumun kanüle edilememesi durumunda şüphelenilmesi gereken bir koroner anomalidir. Burada, akut koroner sendrom kliniği ile başvuran, sol ventrikül sistolik disfonksiyonu olan hastada sol sinüs valsalva görüntülemesi ile tanı konulan bir sol sinüs valsalvadan köken alan sağ koroner arter anomalisi olgusunu sunuyoruz.
Anomaly of Right Coronary Artery Arising from Left Sinus Valsalva in a Patient Applied with Acute Coronary Syndrome and Mild Left Ventricule Dysfunction
Coronary artery anomalies are the clinical conditions which should be considered in differential diagnosis since they can be reason of chest pain, dyspnea, myocardial ischemia, heart failure, ventriculary arrhytmia and sudden cardiac death. Right coronary artery arising from contralateral sinüs valsalva is a type of coronary anomaly and should be suspected during coronary angiography if right coronary ostium can not be cannulated with standard catheters and manipulations. In this case, we report a patient with an anomaly of right coronary artery arising from left sinüs valsalva, who applied with acute coronary syndrome and left ventricule systolic dysfunction and was diagnosed via imaging of left sinüs valsalva.
