Akut Nekrotizan Pankreatitin Nadir Bir Komplikasyonu Olan Gastroduodenal Arter Psödoanevrizmasının Tanı ve Embolizasyonu
Psödokist ve walled-off pankreatik nekroz, akut pankreatitin lokal ve sıklıkla cerrahi müdahale gerektiren geç dönem (>4 hafta) komplikasyonlarıdır. Pankreatitin vasküler komplikasyonları morbidite ve mortalitenin önemli bir nedenidir. Arteriyel komplikasyonlar arasında doğrudan arteriyel erozyon veya psödoanevrizma ve visseraliskemi yer alır. Pankreas psödoanevrizması pankreatik veya peripankreatik arterin bir psödokiste erozyonundan kaynaklanan nadir bir vasküler pankreatit komplikasyonudur. Tedavi edilmezse ölümcül komplikasyonlara yol açabilir. Olgumuzda, akut nekrotizan pankreatitin nadir görülen ve kolaylıkla gözden kaçabilen bir komplikasyonu olan gastroduodenal arter psödoanevrizmasının görüntüleme bulgularını ve pankreatitli hastalarda bu tür komplikasyonların tedavisinde endovasküler girişimin önemini sunuyoruz.
Diagnosis and Embolization of Gastroduodenal Artery Pseudoaneurysm as a Rare Complication of Acute Necrotizing Pancreatitis
Pseudocyst and walled-off pancreatic necrosis are local and often late-stage (> 4 weeks) complications of acute pancreatitis. Vascular complications of pancreatitis are an important cause of morbidity and mortality. Arterial complications include direct arterial erosion or pseudoaneurysm and visceral ischemia. Pancreatic pseudoaneurysm is a rare complication of vascular pancreatitis caused by erosion of the pancreatic or peripancreatic artery in a pseudocyst. If left untreated, it can lead to fatal complications. In our case, we present the imaging findings of gastroduodenal artery pseudoaneurysm, a rare and easily overlooked complication of acute necrotizing pancreatitis, and the importance of endovascular intervention in the treatment of such complications in patients with pancreatitis.
