Yanığa bağlı infeksiyonlu hastalarda antibiyotik kullanımı
Yanıklı hastalarda sistemik inflamatuar yanıt çok şiddetlidir. Bu yüzden infeksiyon kriterleri diğer hastalardan farklı olarak değerlendirilmelidir. Öte yandan; yanık yarası genellikle kolonize olduğundan, bariz infeksiyon bulguları yoksa, sürüntü örneğindeki üremeler infeksiyon işareti olarak kabul edilmemelidir. Kolonize mikroorganizmaların invazyonunu önlemek için topikal antibiyotik kullanımı önerilir. Sistemik antibiyotik tedavisine, infeksiyon tanısı kesinleştikten sonra ampirik başlanmalıdır. Bu yüzden, her ünitenin kendi direnç durumunu bilmesi gerekir. Ampirik tedavi mümkün olduğunca, antistafilokoksik ve antipsödomonal olmalıdır. Ampirik antifungal tedaviye başlamak için risk faktörlerinin çok iyi bilinmesi önemlidir. Yanıklı hastalarda antibiyotik farmakokinetiği ve farmakodinamiği diğer hastalardan farklıdır ve uygun antibiyotik dozu ve serum düzeylerinin sağlanması zordur. Mümkün olduğunca, serum konsantrasyonlarının takip edilerek antibiyotik tedavisinin sürdürülmesi önerilir.
Antibiotic usage in patients with burn infection
Systemic inflammatory response is too severe in patients with burns. So, criteria for infection should be considered different from other patients. On the other hand, burn wound usually colonized with bacteria. So, positive culture results of swab sample should not be considered as an infection without obvious signs of infection. The use of topical antibiotics to prevent invasion of colonizing microorganisms is recommended. Systemic antibiotic therapy should be initiated empirically after the diagnosis is definitely done. Therefore, each unit has to know the status of their antimicrobial resistance. Empirical treatment regimen as much as possible, include antistaphylococcal and antipseudomonal antibiotics. Empirical antifungal therapy to begin, the risk factors is important to know. The pharmacokinetics and pharmacodynamics of antibiotics in patients with burns is different from other patients. Therefore, it’s difficult to ensure appropriate antibiotic dose and serum levels. Whenever possible, serum concentrations of antibiotics should be followed during the therapy.
