İki yıllık süreçte yapılan modifiye hodge testi sonuçlarının irdelenmesi
Antibiyotiklere çoklu direnci olan Enterobacteriaceae suşları ile oluşan infeksiyonların tedavisinde karbapenemler tercih edilmekte; ancak son yıllarda Klebsiella pneumoniae karbapenemazı (KPC) üreten suşlar ile infeksiyonlar da görülmektedir. Direnç tespiti için altın standart moleküler yöntemler olmakla birlikte bu yöntemlerin çeşitli dezavantajları bulunmaktadır. Bu yüzden Clinical Laboratory Standards Institute (CLSI) KPC’lerin tespitinde fenotipik yöntem olan “cloverleaf” testini (modifiye Hodge testi) önermektedir. Çalışmada, Başkent Üniversitesi Adana Uygulama ve Araştırma Merkezi Mikrobiyoloji Laboratuvarında modifiye Hodge testi uygulanmış ve sonuçları pozitif bulunmuş olan klinik örnekler retrospektif olarak değerlendirilmiştir. Çalışmaya Temmuz 2009-Mart 2011 tarihleri arasında çeşitli klinik örneklerden izole edilen karbapenemaz üretimi şüpheli 952 adet Enterobacteriaceae izolatı dahil edilmiştir. CLSI standartlarına göre modifiye Hodge testi yapılmış ve 13 izolatta (% 1.4) pozitif olarak saptanmıştır. Yarı otomatize sistem kitleri kullanılarak izolatların 8’i (% 62) K.pneumoniae, 2’si (% 15) Escherichia coli, 2’si (% 15) Enterobacter aerogenes, 1’i (% 8) Klebsiella oxytoca olarak tanımlanmıştır. Bu suşların 5’i idrar, 4’ü kan, 2’si doku, 1’i batın içi ve 1’i plevra sıvısından izole edilmiştir. Hodge testi pozitif bulunan suşların ikisi (E.aerogenes, E.coli) imipenem ve meropeneme disk difüzyon yöntemi ile duyarlı bulunmuştur. Diğer izolatlarda en az bir karbapenem direnci saptanmıştır. Çalışmamızın sonucunda, karbapenem grubu antibiyotiklerin kullanılması gereken klinik durumlarda tedavinin başarılı olabilmesi için fenotipik yöntemlerle karbapenemaz varlığının araştırılmasının gerektiği düşünülmüştür.
Evaluation of modified hodge test results performed in two years period
Although carbapenems are preferred for treatment of multidrug resistant Enterobacteriaceae, infections with Klebsiella pneumoniae carbapenemase (KPC) producing strains are encountered in recent years. Despite molecular methods are gold standard for resistance detection, they have some disadvantages. For this reason Clinical Laboratory Standards Institute (CLSI) recommends cloverleaf test (modified Hodge test) for KPC detection as a phenotypic method. This is a retrospective study on strains with positive Hodge test results in Baskent University Adana Application and Research Center since 2009 July to March 2011. 952 Enterobacteriaceae isolates were included. Hodge test was positive in 13 of 952 isolates (1.4 %) according to CLSI standards. These 13 isolates were identified as 8 (62 %) K.pneumoniae, 2 (15 %) E.coli, 2 (15 %) Enterobacter aerogenes, 1 (8 %) Klebsiella oxytoca by using semi automated system kits. Five of the isolates were from urine samples, 4 from blood, 2 from tissue, 1 from periton fluid samples and 1 from pleural effusion. Two of the positive samples with Hodge test (E.aerogenes, E.coli) were found sensitive to imipenem and meropenem with disk diffusion methods. The others were resistant to at least one carbapenem by the same method. We believed that phenotypic methods for carbapenemase production will be helpfull to decide the use of carbapenem group antibiotics for therapy of the multidrug resistant Enterobacteriaceae strains.
