KRONİK HEPATİT C İNFEKSİYONU OLAN HASTALARDA SERUM HCV-RNA DÜZEYİ İLE KARACİĞERİN HİSTOPATOLOJİK ÖZELLİKLERİNİN KARŞILAŞTIRILMASI
Hepatit C virüsü (HCV); uzun yıllar non-A, non-B hepatiti olarak bilinmiş, en sık kan transf üzyonu yapılan hastalarda ayrı bir klinik antite olarak görüldüğünden 1975te aynı zamanda post-transf üzyon hepatit nedeni olarak tanımlanmıştır. Bu çalışmanın amaçları; HCV RNA düzeyi ve alanin aminotransf eraz (ALT) değerlerinin karaciğerdeki steatoz,histolojik aktivite indeksi ve f ibrozis ile aralarındaki ilişkinin değerlendirilmesi ve aynı zamanda kronik hepatit Cde f ibroz ve steatozun durumunu araştırmaktır. Karaciğer biyopsisi ile doğrulanmış kronik HCV tanısı almış ancak herhangi bir tedavi almamış 100 hasta çalışmaya alınmış- tır. Hastaların serum HCV RNA ve ALT değerleri ile karaciğer histopatolojik özellikleri karşılaştırılmıştır. Çalışmaya 63 kadın ve 37 erkek hasta alınmıştır. Yaş ortalaması 55.48±11.04 olarak bulunmuştur. Hastaların steatoz değer- lendirilmesi Kleiner skorlamasına göre yapılmıştır. Hastaların % 42sinin steatoz evresi 0, % 41inin steatoz evresi 1, % 10unun steatoz evresi 2, % 7sinin steatoz evresi 3 olarak saptanmıştır. Histolojik aktivite indeksi Ishak skorlamasına göre (0-18) değerlendi- rilmiştir. Çalışmamızda haf if kronik HCV hastalığı, % 70.7 ile en çok görülen karaciğer hastalığı olmuştur. Hastaların karaciğer f ibrozis skorlaması da Ishak skorlamasına göre yapılmış ve en sık % 74.7 ile haf if f ibrozis görülmüştür. Serum HCV RNA düzeyleri ile steatozis arasında negatif korelasyon bulunurken serum ALT düzeyi ile histolojik aktivite indeksi ve f ibrozis skoru arasında pozitif ilişki bulunmuştur. HCV RNA viral yükün karaciğer histolojisini etkileyen bir parametre olmadığı, karaciğer f ibrozisine ilerlemesinde steatozun bir katkısının olmadığı belirlenmiştir.
The Comparison of Serum HCV-RNA Levels and Histopathological Features of Patients with Chronic Hepatitis C Infection
Hepatitis C virus (HCV), f ormerly known as non Anon B hepatitis, was also described as posttransf usion hepatitis as a separate clinical entity in 1975 because it was f requently reported in patients who received blood transf usions. The aims of this study are to evaluate the relationship between the serum levels of HCV RNA and alanine aminotransf erase (ALT) with steatosis,histologic activity index and f ibrosis in liver and the same time to research the condition of f ibrosis and steatosis in chronic hepatitis C. Patients who diagnosed with chronic HCV, conf irmed by liver biopsy and have not received therapy were enrolled in the study. The patients serum HCV RNA and ALT levels were compared with liver histopathological f eatures. One hundred patients including 63 women and 37 men were included in the study with an average age of 55.48 ±11.04 years. Steatosis was assessed according to Kleiner scoring system obtaining the f ollowing scores: 42 % of the patients steatosis were f ound to have scored 0, 41 % of them scored 1, 10 % scored 2, and 7 % scored 3. Histological activity index was evaluated according to Ishak scoring system. Mild chronic HCV was the most f requent liver disease with the ratio of 70.7 %. Patients liver f ibrosis scores were also evaluated according to Ishak scoring system and mild f ibrosis was seen in 74.7 % of the patients. While there was a negative correlation between serum HCV RNA levels and steatosis, positive correlation was detected bet- ween serum ALT levels and histological activity index and liver f ibrosis scores. HCV RNA viral load was not a parameter eff ecting liver histology. There was no impact of steatosis on the progression of liver f ibrosis.
