1999-2006 yılları arasında dışkı kültürlerinden izole edilen 439 Shigella kökeninin tür dağılımı ve antibakteriyel direnç durumları
Shigella türleri tüm dünyada en sık gastroenterit etkenleri arasındadır. Dirençli kökenler ile ortaya çıkan infeksiyonlarda tekrarlama sıklığı ve kompikasyonlar, empirik tedavide uygun antibiyotik seçiminin önemini artırmaktadır. Bu çalışmada bakteriyoloji laboratuvarımıza 1999-2006 yılları arasında gönderilmiş ve Shigella kökenleri üremiş dışkı örneklerinin sonuçları retrospektif olarak incelenmiştir. İzole edilen 439 adet Shigella kökeninin 16’sı S.boydii (% 4), 19’u S.dysenteriae (% 4), 108’i S.flexneri (% 25) ve 296’sı S.sonnei (% 67)’dir. Trimetoprim/sülfametoksazol, ampisilin ve kloramfenikole direnç oranları sırasıyla % 69, % 24 ve % 12 olarak saptanmıştır. Siprofloksasin ve sefotaksime direnç saptanmamıştır. Bölgemizde ampisilin ve trimetoprim/sülfametoksazolün ampirik tedavideki rollerini kaybettikleri görülmüştür. Bununla birlikte izole edilen kökenlerde siprofloksasin ve üçüncü kuşak sefalosporinlere direnç görülmemesi oldukça sevindiricidir.
Species distribution and antibacterial resistance patterns of 439 Shigella spp. strains isolated from stool cultures between 1999-2006
Shigella species are among the most common gastroenteritis agents worldwide. Relapses and complications in infections with resistant strains increase the importance of appropriate antibiotic selection in treatment. In this study results of all stool cultures yielding Shigella performed in our hospital’s bacteriology laboratory between 1999 and 2006 were investigated retrospectively. Of 439 Shigella strains 16 (4 %) were S.boydii, 19 (4 %) S.dysenteriae, 108 (25 %) S.flexneri and 296 (67 %) S.sonnei. Rates of resistance to trimethoprim/sulfamethoxazole, ampicillin and chloramphenicol were 69 %, 24 % and 12 %, respectively. Resistance to ciprofloxacin and cefotaxime was not detected. As a result, it was seen that in our region ampicillin and trimethoprim/sulfamethoxazole lost their roles in the empirical treatment. By the way it is pleasing that no resistance was detected to ciprofloxacin and cefotaxime.
