Total larinjektomi sonrası neofarinksin ultrasonografi ile değerlendirilmesi

Amaç: Total larinjektomi sonrası neofarinksin değerlendirilmesinde ultrasonografinin tanı değerini, bilgisayarlı tomografi ile karşılaştırmalı olarak araştırmak. Gereç ve Yöntem: Median yaşı 50 (30-75) olan 21 total larinjektomili olguya, ardışık olarak boyun ultrasonografisi (7.5 MHz’lik lineer transdüser ile) ve kontrastlı boyun bilgisayarlı tomografisi yapıldı. Neofarinksin duvar katlarının ayırdedilmesi ve ultrasonografinin nüksü saptamadaki başarısı bilgisayarlı tomografi ile karşılaştırmalı olarak değerlendirildi. Bulgular: İki (%9) hastada neofarinkste nüks ve servikal lenf nodu metastazı her iki tetkikle de saptandı, tanılar histopatoloji sonuçları ile doğrulandı. Neofarinks duvarının ayrıntılı değerlendirilmesinde duyarlılık bilgisayarlı tomografi ve ultrasonografide %100 idi. Seçicilik bilgisayarlı tomografi için %100, ultrasonografi için %94.7 olarak saptandı. Sonuç: Total larinjektomi sonrası neofarinks nükslerinin saptanmasında ultrasonografinin duyarlılık ve seçiciliği bilgisayarlı tomografi ile benzerdir. Ultrasonografinin ucuz ve kolay ulaşılabilir olması nedeniyle total larinjektomi sonrası hasta takibinde faydalı olabileceği söylenebilir.

Evaluation of fneopharynx by ultrasonography after total laryngectomy

Aim: To assess the diagnostic value of ultrasonography for detecting recurrences of neofarynx after total laryngectomy in comparison with computerized tomography. Materials and Methods: Twenty-one patients underwent total laryngectomy for carcinoma between October 2002 and December 2003 were included this prospective study. Neopharynx and neck were evaluated with ultrasonography and computed tomography, independently by two radiologists. Results: Two patients had local recurrences in neopharynx and in neck, in their follow-up periods. For ultrasonography, sensitivity and specificity in demonstrating recurrences were 100% and 94.7%, respectively. These rates for computed tomography were 100%. Conclusion: Ultrasonography is usefull and cost-effective for detecting of recurrences after total laryngectomy.
