Postoperatif ağrı tedavisinde epidural klonidin ile fentanilin karşılaştırılması

ÖZET Biz bu çalışmamızda postoperatif ağrı tedavi¬sinde alfa-2-adrenerjik reseptör agonisti olan klo¬nidin ve fentanil arasındaki analjezik, solunumsal ve hemodinamik etkilerini karşılaştırmayı planla¬dık. Açık prostatektomi operasyonu geçiren ve ağrı yakınması olan A5A-I ve II grubundan 30 has¬ta çalışmaya alındı. Epidural anestezi uygulanarak öpere edilen hastalar sensitif ve motor bloktan kurtulduktan sonra, War hastalık 3 gruba ayrıldı. Toplam volüm W mi olacak şekilde, epidural ka-teterden 1.gruba 2mcg/kg klonidin, 2.gruba l.5mcg/kg fentanil ve 3.gruba plasebo uygulandı. Hiçbir hastada duyusal, motor veya refleks fonksiyonlarda herhangi bir değişiklik ile bulantı-kusma, kaşıntı ve solunum depresyonuna rastlan¬madı. Sonuç olarak, postoperatif ağrı tedavisinde kullanılan opioidlerin oluşturdukları yanetkileri gözönüne alındığında, klonidin uygulamasının daha güvenilir bir yöntem olduğunu söyleyebili¬riz.

Comparison of epidural clonidine an fentanyl for the treatment of postoperative pain

In this study, at postoperative pain manage¬ment, we have planned to investigate and compa¬re the analgesic, respiratory and hemodynamics effects of fentanyl and clonidine which alpha-2-adrenergic receptor agonist Thirty patients who, had postoperative pain after open prostatectony operation were included in the study. All patients were in ASA-I, II. After improving of motor and sensorial effects of epidu¬ral anesthesia, patients were divided in three gro¬ups. Each group included 10 patients. We have applied 2 mcg/kg Clonidine ,1.5 mcg/kg fentanyl and placebo in total 10 mg dose at first, second and third group respectively. We have not seen any changes at sensorial, motor and reflex functions. Also we have not seen respiratory depression, vomiting and pruritis. As a conclusion, we suppose that clonidine seems to be safer than opioids which cause much adverse effects at postoperative pain management.
