Nazal rekonsrüksiyonda deneyimlerimiz ve modifiye nazal subünitelerin rolü

Bu çalışmada Haziran 1984 ile Mayıs 2002 tarihleri arasında burun rekonstrüksiyonu yapılan 726 hasta retrospektif olarak değerlendirilmiştir. Altmış dokuzu kadın, 57'si erkek olan 126 olguya 143 nazal rekonstrüksiyon uygulanmıştır. Bu olguların 97'si Bazal hücreli kanser (BCC), 22'si yassı hücreli kanser (SCC), 4'ü hemanjiom, 2'si travmaya bağlı doku defekti, l tanesi de melanom nedeniyle opere edildi. Yüz yirmi altı olguda defektleri kapatmak için 41 tam kalınlıkta deri grefti (TKDG), 39 nazolabial flep, 28 alın flebi, 14 bilobe flep, 9 transpozisyon flebi, 7 primer onarım, 4 nazal dorsal flep, 1 serbest radial önkol flebi kullanıldı. Bu olguların 54'ünde modifiye nazal subünitelere uyularak rekonstrüksiyon gerçekleştirildi. Estetik ve fonksiyonel sonuçlar mükemmel, iyi ve orta olarak derecelendirildi. Modifiye subünitelere göre opere edilen bu 54 olguda sonuçlar %80 mükemmel, %11 iyi ve %9 orta olarak belirlendi. Olgular nazal subünitenin modifikasyonunu gösterecek şekilde seçildi.

Our experiences in nasal reconstruction and the role of modified nasal subunits

In this study, a retrospective analysis of 126 patients who underwent nasal reconstruction between June 1984 and May 2002 was evaluated. 126 patients consist of 57 men and 69 women underwent 143 nasal reconstructions. These cases were operated for 97 basal celi carcinomas (BCC), 22 squamous celi carcinomas, 4 hemangiomas, 2 traumatic tissue defects, and l melanoma in 126 cases, for closure defects were used 41 full-thickness skin grafts (FTSG), 39 nasolabial fold flaps, 28 forehead flaps, U bilobed flaps, 9 transposition flaps, 7 primer closures, 4 nasal dorsal advancement flaps, and one free radial forearm flap. in the 54 patients, reconstructions were performed according to modified nasal subunit principles. The aesthetic and functional results were graded as excellent, good, and fair. The results for the 54 patients who underwent modification of the subunit were excellent in 80 percent, good in 11 percent, and fair in 9 percent. Cases were selected to illustrate nasal subunit modification.
