SİGARA VE POSTDURAL FONKSİYON BAŞ AĞRISI

Amaç: Postdural ponksiyon baş ağrısı (PDPB) spinal anestezinin iyi bilinen bir komplikasyonudur. Yaş, cinsiyet, gebelik, öncekiPDPB öyküsü, iğne çapı ve şekli, uygulayıcının klinik deneyimi gibi birçok faktör oluşumunda etkilidir. Fakat sigara maruziyetiile PDPB oluşumu arasındaki ilişki ile ilgili literatürde çok az bilgi mevcuttur. Biz, sigaranın koagülasyon kaskadı üzerine olanetkilerini göz önünde bulundurarak, sigara kullanımı ile PDPB oluşumu arasındaki olası ilişkiyi incelemeyi amaçladık.Yöntem: Çalışma için spinal anestezi altında sezaryen operasyonu geçirmiş olan 254 gebe kadının verileri retrospektif olaraktoplandı. Hastaların postoperatif 1 haftalık kayıtları incelendi, baş ağrısı sıklıkları, başlangıç zamanları ve süreleri dosya kayıtlarındanalınarak not edildi.Bulgular: Çalışma için 254 hastanın verileri toplandı. Hastalar sigara içiciliği durumuna göre Grup S (sigara içenler,n=100) ve Grup N (sigara içmeyenler n=154) olarak iki gruba bölündü. Grup S'de 12(%12) ve Grup N'de de 12(%7.8) hastadaPDPB gelişmişti. İki grup arasında anlamlı bir fark yoktu.Sonuç: Sigara içiciliği PDPB gelişme riskini etkilemiyor gibi görünmektedir. Konu ile ilgili mevcut literatürdeki bilgi azlığınıgöz önüne alarak, bu alanda daha fazla çalışma yapılabilir. Yapılacak olan çalışmalar sigara maruziyetinin miktarı hakkında dabilgi içermelidir

SMOKING AND POSTDURAL PUNCTURE HEADACHE

Objective: Postdural puncture headache (PDPH) is a well known complication of spinal anesthesia. Many factors like age,sex, pregnancy, previous history of PDPH, needle size and shape, clinical experience of the operator are involved in its development.However, there is little information about the relationship between tobacco exposure and PDPH in the current literature. Weaimed to investigate the possible relationship between smoking habit and PDPH development, taking into consideration theeffects of tobacco on coagulation cascade.Method: Data of 254 pregnant female patients underwent cesarean delivery under spinal anesthesia were retrospectivelycollected for the study. Postoperative 1 week records of the patients were evaluated and headache incidences were noted, as wellas onset times and durations from the patient files.Results: Data of 254 patients were collected for the study. Patients were divided into two groups regarding their smokingstatus as Group S (smokers, n=100) and Group N (non-smokers, n=154). Twelve (12%) patients in the Group S and 12 (7.8%) inthe Group N had experienced PDPH. No significant difference was found between the two groups.Conclusion: Tobacco exposure doesn’t seem to affect the risk of PDPH. More studies can be made in this respect, consideringscarcity of the material regarding the issue in the current literature. Further studies should contain data about the quantity oftobacco exposure
